Self-Certification for Charities
Form for 501(c)(3) organizations which are favorably rated by a watchdog organization, the Better Business Bureau Accredited Charity Directory or participated in the Combined Federal Campaign.
Self-Certification for Educational Institutions
Form for organizations which are 501(c)(3) and listed in the U.S. Department of Education's Database of Accredited Postsecondary Institutions and Programs.
Self-Certification for Other Non-Profit Organizations
Form for 501(c) organizations that serve disabled Veterans and whose status can be confirmed by the IRS or a publicly available service.
Self-Certification for Other Entities
Form for groups or organizations that offer services and/or support to Service Members, Veterans and their families and are endorsed by, or formally recognized by and/or partnered with, a government agency as evidenced by public recognition (i.e. listed on a .gov or .mil Web site).
