  The negative impact of traumatic events can extend beyond those who directly experienced the trauma. Health care professionals who provide services to traumatized individuals are at risk for a phenomenon called compassion fatigue. Experiencing compassion fatigue can lead to impaired social and occupational functioning.

  This presentation will introduce attendees to the concept of compassion fatigue, describe its symptoms and discuss strategies to minimize the negative impact.

  At the conclusion of the workshop, attendees will know how to:

  • Formulate a clear conceptual understanding of compassion fatigue, including how it is similar to and/or different from related constructs
  • Identify the symptoms of compassion fatigue in oneself and others
  • Differentiate between the risk factors and protective factors associated with compassion fatigue

• Technological Updates in the Treatment of Mental Health Conditions (July)

  Using technology designed to improve health care is not a new concept. However, access to evidence-based technology to assist with psychological health care treatment, such as psychotherapy, is limited.

  Without an established standard of care, it is appropriate for providers to use a “null” or waitlist control group in clinical trials. However, in cases where health care providers apply technology-based solutions to an evidence-based practice, they are required to compare these solutions to active treatments.

  One approach, becoming more prominent in medical literature, is to use a non-inferiority trial. The overall goal of this type of trial is to test and demonstrate that an experimental treatment is no worse than the current standard of care. Unfortunately, some reports about using this type of trial reveal confusion or a lack of understanding about the technical and philosophical distinctions of the trial design.

  In this webinar, we will develop the tools needed to evaluate the quality of the evidence base to improve clinical care. We will use trials that incorporate technology to administer psychotherapy compared to the in-office standard of care as a practical case study.

  At the conclusion of this webinar, participants will know how to:

  • Identify the key design elements of a non-inferiority study
  • Interpret the results of a non-inferiority trial
  • Evaluate the credibility of the evidence base for a treatment approach based on non-inferiority designs

• Obesity, Eating Behaviors and Stigma among Service Members (June)

  Despite the fact that a majority of Americans are overweight, individuals who are overweight or obese are frequent targets of stigma and prejudice in multiple domains, including employment, education, health care and portrayal in the media. The psychological consequences of weight stigma include low self-esteem, poor body image, depression, anxiety and suicidal behaviors. Maintaining weight standards across the Defense Department may be a challenge for service members who are overweight or obese and they may experience related stigma and bias.

  This webinar will highlight Defense Department weight standards and explain the construct of weight stigma and its impact on active-duty service members. In addition, it will discuss the Fit4Duty study, which seeks to reduce excess weight gain in the military population, and recommendations for how to address the issue of obesity in health care settings.

  At the conclusion of this webinar, participants will be able to:

  • Interpret the construct of weight stigma and the potentially adverse effect of weight stigma on the psychological functioning of active-duty service members with overweight and obesity
  • Use new strategies to address excess weight among service members without shame or bias

• Facilitating Help-seeking Behavior for Psychological Health Concerns in Service Members (May)

  Service members are often susceptible to increased rates of diagnosable psychological health concerns. This is particularly true in service members who have experienced multiple deployments and/or served in combat roles. Data suggests that relatively few service members with psychological health concerns seek help, which may increase the risk of ongoing symptoms, diminished readiness, and career implications. These barriers to care are present across all service branches, including the National Guard and reserve components, who face additional challenges and potentially reduced access to psychological health care or resources. This review presents research findings about the barriers to care for service members, along with systematic efforts to reduce stigma and facilitate help-seeking behavior. Additionally, the presentation will discuss strategies that health care providers and line leaders can implement to reduce stigma and facilitate help-seeking behavior.

• Prevention of Sexual Abuse of Children (April)

  Child sexual abuse refers to the entire spectrum of sexual crimes and offenses in which children up to age 17 are victims. The U.S. Department of Health and Human Services’ Children’s Bureau report Child Maltreatment 2010 found that 9.2 percent of victimized children were sexually assaulted with children being the most vulnerable to sexual abuse between the ages of 7 and 13. Child sexual abuse victims often feel significant distress and display a myriad of short- and long-term psychological symptoms and developmental delays, and they are at increased risk for experiencing future sexual assaults. The webinar will discuss promising strategies for prevention of child sexual abuse including school-based education, parent education, treatment for victims, treatment for juvenile and adult offenders, law enforcement training, and interviewer training.

• Deployment-related Co-occurring PTSD and Mild TBI in Service Members (March)

  Mild traumatic brain injury (mild TBI) or concussion has been identified as a hallmark injury of the Afghanistan and Iraq wars. This review addresses the impact of mild TBI on the development, course and clinical management of PTSD. Research efforts take into consideration the potential differential impact of PTSD and mild TBI with or without persistent cognitive deficits. Findings have shown the impact of mild TBI on response to existing PTSD treatment interventions, and development and examination of potential treatment augmentation strategies.

  Understanding the epidemiology, diagnostic evaluation and clinical management of common physical symptoms can benefit both physical and psychological health. The goal of this webinar is to share current research and treatment practices related to post-deployment PTSD symptoms, including those attributed to mild TBI.

• Literature Review on Resilience in the Military (February)

  In an attempt to define resilience, researchers have examined adaptation and growth and capacity versus demonstration. Findings have shown that positive adaptation is influenced by factors both outside and inside the work setting. When examining resilience in high-stress occupations, the process includes appraisal of adversity, coping with adversity and seeking help from others in order to achieve positive adaptation. To address the challenges of positive adaptation for those deploying to war zones or other high-stress environments, the Department of Defense implemented pre-deployment training on resilience. The training is based on literature that identified several predictors of resilience in military personnel including quality of sleep, higher unit moral and positive leader behavior.

• Scientific Reviews of Recent Studies on the Treatment of Posttraumatic Stress Disorder (January)

  The Department of Defense (DoD) and Department of Veterans Affairs (VA) collaborated with the Institute of Medicine (IOM) in a two-phased review of prevention, screening, diagnosis, treatment and rehabilitation of posttraumatic stress disorder (PTSD). Phase I assessed DoD and VA collaborative efforts as well as related research studies and clinical trials. In Phase II, IOM gathered additional data from site visits at military treatment facilities (MTFs) and VA facilities and from literature reviews. Using these methods, IOM examined PTSD management systems and identified components to assist DoD and VA in closing gaps in care, improving delivery of care and increasing quality of care. In addition, several MTFs provided valuable insights into evidence-based treatments in areas of psychotherapy and pharmacotherapy, as well as complementary and alternative therapies, such as acupuncture, art therapy and biofeedback.

• Unique Perspective for Women with Traumatic Brain Injury: Gender Differences and Coping Strategies (October)

  The data regarding active-duty servicewomen who have sustained traumatic brain injury (TBI) suggest their experiences, aftereffects and outcomes differ from servicemen. • Animal-Assisted Therapy: An Alternative Treatment for Traumatic Brain Injury Rehabilitation (August)

  Many service members returning from combat deployments are coping with the “invisible wounds of war” including traumatic brain injury (TBI) and posttraumatic stress disorder (PTSD). These wounds often negatively impact quality of life and community reintegration. The Walter Reed National Military Medical Center Service Dog Training Program assists patients with TBI, PTSD and other psychological injuries through the clinically-based, goal-directed complementary treatment intervention of Animal-assisted therapy. Speakers will address the healing power of the human-animal bond, how patients are taught to train mobility service dogs, how these skills improve TBI and PTSD symptoms, and current research initiatives.

• Using the Performance Triad for Optimal Traumatic Brain Injury Recovery (July)

  In 2013 Army Medicine launched the Performance Triad to maintain, restore and improve health through making informed choices. This holistic approach to health systems focuses on three key foundations that influence a person’s health – sleep, activity and nutrition. Under the model of maintaining, restoring and improving health, the Performance Triad offers a proactive approach to wellness; this model can empower patients with mild traumatic brain injury (TBI) to actively engage in their recovery by integrating informed choices with the supportive care of their providers. The Army Ready and Resilient Campaign is an example of the Performance Triad approach.

  Service members and veterans with a TBI often experience disruptions in sleep patterns and restrictions in both physical and cognitive activities during the recovery period. Dietary factors may influence recovery and should be carefully considered in patient management. Health care providers, therefore, should pay particular attention to sleep, exercise (or activity level) and nutrition in managing patients with mild TBIs. Providers can effectively apply the key components of the Performance Triad to optimize patient recovery.

  This webinar will review the essential components of the Performance Triad and describe how clinicians can apply this model to enhance patient outcomes from TBI.

  Service members and veterans who experience a mild traumatic brain injury (TBI) often have persistent cognitive symptoms. • Management of Headache Following Concussion/Mild TBI: Guidance for Primary Care Management in Deployed and Non-Deployed Settings (April)

  More than 339,000 service members sustained a traumatic brain injury (TBI) between 2000 and 2015 with approximately 82 percent of these classified as mild TBI, also known as concussion (Defense and Veterans Brain Injury Center, 2016). Headache is the most common symptom reported following a concussion (Lucas, Hoffman, Bell, & Dikmen, 2014). In a study of Operation Enduring Freedom/Operation Iraqi Freedom veterans, 74 percent reported post-traumatic headaches (PTH) occurring within 30 days of sustaining a concussion (Theeler, Flynn, & Erickson, 2010).

  The presentation will review the newly released Clinical Recommendation which offers guidance for the primary care management of PTH in deployed and non-deployed settings. The speakers will address the characteristics of four of the most common types of PTH and review non-pharmacologic and pharmacologic treatment recommendations for each type. In addition, they will address when specialty referral may be indicated. Practical resources, including the clinical recommendation, provider training slides, companion clinical tool and patient education materials, will be available for download.

• Management of Sleep Disturbances Following Concussion (March)

  Nearly 340,000 service members sustained a traumatic brain injury (TBI) between 2000 and 2015 with 82.5 percent of these classified as mild TBI, also known as concussion (Defense and Veterans Brain Injury Center, 2016). Mathias & Alvaro (2012) showed that as many as 50 percent of people who sustained a concussion suffered from a sleep disturbance. Additionally, in a 2008 Department of Defense survey of Operation Iraqi Freedom service members, 92.9 percent of those surveyed with a TBI history endorsed fatigue (Hoge et al., 2008). Sleep disturbances and fatigue can lead to worsening symptoms such as decreased cognition, pain, irritability and ultimately affect return to work.

  Clinically, sleep problems may co-exist with headache, somatic pain, and other neurological symptoms, and diagnosis and treatment can be challenging for providers. This presentation will address the assessment and management of concussion-associated sleep disturbances and fatigue. The speakers will present recent research and discuss ways to enhance quality of life and function in individuals who are experiencing post-concussion sleep dysfunction and fatigue.

  Nearly four million Americans are affected by sport or recreational traumatic brain injuries annually. Winter sports (e.g., ice hockey, skiing, sledding) are often fast-paced, high-velocity activities that can lead to serious injury including concussion, or mild traumatic brain injury. Surprisingly, head injuries in winter sports have not been studied as frequently as those in football or soccer. The most studied winter sport is ice hockey. This webinar will provide an overview of winter sports injury with an emphasis on ice hockey research designed to understand injury mechanisms, dispel concussion myths, and present novel opportunities for preventing injury.

• Do Head Injuries Cause Chronic Traumatic Encephalopathy? (January)

  Contact sports have long been suspected of causing prolonged or permanent neurologic injury in some athletes; the 2005 findings of unusual brain pathology in a retired NFL football player magnified concern for this association. The brain pathology involved accumulation of the protein tau in masses called tangles or threads. To date, more than 150 cases of chronic traumatic encephalopathy (CTE) have been confirmed by autopsy, with the vast majority being athletes in contact sports. Records show these individuals suffered a range of cognitive, emotional, behavioral, and motor symptoms. In some cases, co-occurring neurological conditions were present that could also account for observed symptoms.

  The distribution of accumulated tau masses, the criteria for diagnosis, and the clinical presentation of CTE remain controversial among researchers. At this time, there is no consensus regarding the clinical presentation of CTE that could enable diagnosis in living persons. There is no fluid or neuroimaging biomarker validated for diagnosis, although some imaging approaches show promise in their ability to differentiate different neurological conditions and normal aging. The literature on CTE is also limited by the strong influence of a few individual researchers, whose studies may be influenced by selection bias.

  The webinar will address the current neuropathological diagnostic criterion for CTE and highlight recent findings that support or refute the association of CTE with multiple concussions.

  At the conclusion of this webinar, participants will be able to:

• Detrimental Effects of Blue Light from Electronics on Sleep (December)

  Light isn’t just for vision. In addition to enabling us to see, light is the major synchronizer of our bodies’ circadian rhythms to the local time on earth. Light can also induce an acute alerting effect on people. Lighting characteristics affecting vision are different than those affecting our biological rhythms and our acute alertness. Different types and levels of light can affect a person's ability to see clearly, identify people and objects, and drive safely.

  Certain types of light applied at certain times of the day can increase sleep efficiency of older adults and reduce symptoms of seasonal affective disorder, or SAD, felt by many people during winter months. If applied at the wrong time, light can lead to circadian rhythm disruption, which has been linked to increased risk for diseases and disorders. How and when lighting can be used to promote health and well-being of those suffering from circadian disorders will also be discussed.

• Using Meta-analysis to Determine the Most Effective Treatments for Posttraumatic Stress Disorder (October)

• Therapeutic Risk Management of the Suicidal Patient (September)

• The Impact of Sexual Assault and Treatment Options (August)

  According to a Department of Health and Human Services, Children’s Bureau fiscal year 2012 report, U.S., state and local child protective services received an estimated 3.4 million referrals of children being abused or neglected. Of these child victims, 9 percent were victims of sexual abuse. The National Intimate Partner and Sexual Violence Survey 2010 Summary Report identified that one in two women (44.6 percent) and one in five men (22.2 percent) experienced sexual violence other than rape in their lifetime. Victims of sexual assault often experience a range of psychological and emotional disorders including rape trauma syndrome, depression, anxiety, and post-traumatic stress. The psychological and emotional trauma these victims experience can also manifest itself in physical reactions such as stomach aches, headaches, and back problems. Well-researched trauma-informed treatment approaches and applicable prevention methods have shown positive outcomes for reducing the impact of the abuse.

• Alcohol Misuse in the Military: Screening Brief Intervention and Referral to Treatment (July)

  According to the Department of Defense 2011 Health Related Behavior Survey, approximately 33% of active duty service members reported binge drinking in the past 30 days, higher than the civilian estimate of 27%. Several interventions for alcohol use disorders exist, but many of these approaches are too intense for sub-threshold alcohol misuse. The Screening, Brief Intervention, and Referral to Treatment (SBIRT) model, promoted by the U.S. Preventive Services Task Force in 2013, is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with alcohol use disorders. The SBIRT model enables healthcare professionals to systematically screen patients with the goal of preventing the negative effects of alcohol use among service members and beneficiaries.

• Physical Symptoms and Mental Health: Assessment, Evaluation and Management of Common Symptoms (February)

  Common physical symptoms such as pain, fatigue, insomnia and gastrointestinal complaints account for half of all outpatient visits. However, numerous studies reveal that diagnostic test results are often normal and the symptoms remain unexplained. Depression and anxiety often accompany physical symptoms in which case co-management of both physical and psychological conditions is important. Understanding the epidemiology, diagnostic evaluation and clinical management of common physical symptoms can benefit both physical and psychological health.

• The Well-being of Military Children: Augmenting Clinical Care with Web- and Mobile-based Tools (March)

  Research on the psychological health of military children indicates that frequent transitions such as moving, family member separations and the cumulative effects of multiple deployments can cause psychological distress, which impacts the well-being of parents and children. Although military children are generally resilient, many are coping with increased levels of anxiety, stress, depressive symptoms, suicidal ideation and behavioral problems.

  Age-appropriate psychoeducation web-based programs for children have been found to assist with teaching coping strategies and as a result of online community peer support improve their behavioral health. Discussion will include a review of the Military Kids Connect website, a technology-based resource for military preschoolers to teens, which may serve to augment clinical care practices and to improve how health care providers interact with military children.

• Clinical Benefits of Telehealth Technology in Behavioral Health Care (February)

  In today’s world of technology and connectivity, outpatient behavioral health care services are delivered in much the same manner as they have for the last 65 years; a patient takes time off work or away from family, commutes to the provider’s office, and meets with the provider individually or in a group, in a shared, private space.

  This traditional model of care may impose barriers that increase the difficulty and challenges for those seeking treatment, and may lead some to avoid behavioral healthcare completely. Couple this with the nearly 80 million Americans living in underserved areas (http://www.hrsa.gov/shortage/), and an individual’s access to high-quality behavioral-health services becomes a key issue facing the behavioral health field.

  Telemental health care, the delivery of behavioral health care to a location where the provider is not physically present, is one way to enable access to care. Research consistently supports the use of technology such as video teleconferencing to deliver effective evidence-based care. This webinar focuses on the clinical benefits of remote health care, both to the patient and to the therapy process.

• Application of Behavioral Health Technology Tools in the Clinical Care of Mild Traumatic Brain Injury (January)

  The clinical practice guidelines produced by the departments of Defense and Veterans Affairs provide a framework for ensuring evidence-based care for patients with mild traumatic brain injury (TBI). This webinar will demonstrate two mobile applications produced by the National Center for Telehealth & Technology that offer providers evidence-informed tools for treatment and engagement of patients with mild TBI. The presenter will review some empirical findings that support the use of these apps and will demonstrate how to incorporate these tools into current clinical practice. In addition, he will highlight several emerging tools that may have promise for future use with this population.

  • Compare innovative, behavioral-health technology tools and describe some of the empirical evidence for their integration in clinical care with mild TBI patients.

• Defense and Veterans Brain Injury Center “Head to Head” Study: A Psychometric Comparison of Brief Computerized Neuropsychological Assessment Batteries (December)

  With more than 300,000 service members diagnosed with traumatic brain injury (TBI) since 2000, a need for fast and easy assessment of cognitive functioning has arisen. Numerous computerized neurocognitive assessment tools (NCATs) have emerged from this need. Companies creating these tests often tout them as suitable alternatives to traditional pencil and paper tests. However, emerging research suggests this may not be true, and the issue is not as straightforward as once believed.

  Investigators at Fort Bragg, North Carolina recently completed a two-phase study of the psychometric properties of four NCATs: ANAM4, CNS-Vital Signs, CogState and ImPACT. The first phase investigated the test-retest reliability of the NCATs by comparing examinees’ scores over a 30-day interval. The second phase investigated the validity of the NCATs by comparing the performance of healthy service members and service members with acute mild TBI to their performance on traditional tests. In the context of this study, this webinar will present the state of the literature regarding NCATs, their clinical utility and future directions.

• ICD-10 Coding Guidance for Traumatic Brain Injury within the Military Health System (November)

  All programs covered by the Health Insurance Portability and Accountability Act were mandated to transition to ICD-10 code sets as of Oct. 1, 2015. This transition accommodates dramatic changes in the practice of medicine and provides the flexibility to adapt as medicine changes. The new code set also provides a significant increase in the specificity of reporting, allowing more information to be conveyed in a code.

  This webinar will address how the ICD-10-CM codes use seven alphanumeric characters to represent illnesses and conditions and present the commonly used codes for traumatic brain injury (TBI). Accurate ICD-10 coding is critical; proper coding provides a detailed clinical picture of a patient population, contributes to quality outcomes and standards of care, permits reimbursements for clinical services and prevents over- or under-billing for services. In addition, it permits oversight of population health within a military treatment facility, a region or the entire Military Health System (MHS) as well as helps anticipate demand for future TBI services.

  The Centers for Disease Control and Prevention (2014) reports that in one year alone traumatic brain injuries (TBI) accounted for approximately 2.2 million emergency department visits, 280,000 hospitalizations, and 50,000 deaths. Those who survive a TBI may experience short- and long-term effects such as alterations in thinking, sensation, language, behavior and emotions, which affect the entire family. Family members are often at a loss to understand problematic behaviors and assist their loved one in daily routines and during times of stress.

  This webinar will address family challenges and adjustment following TBI. Discussions will include teaching caregivers coping mechanisms and specific problem-solving strategies associated with optimal adjustment. • Translational Neurorehabilitation Outcomes: Research Trends and Person-Centered Care (June)

  Trauma care and acute care have significantly improved the survival of and reduced disability in individuals who have sustained moderate to severe traumatic brain injury (TBI). However, post-hospital care has been less well defined or validated as an intervention for this population. This webinar addresses research which supports the neurological continuum of care from the neurological intensive care unit through post-hospital care to return home.

  Current research substantiates that post-hospital care leads to positive results and ultimately decreases disability in individuals with extensive injury. Statistical analyses such as the Rasch statistical technique have the potential for determining the level of rehabilitation intervention. This technique reveals that Instrumental Activities of Daily Living tend to be the most difficult skills to rehabilitate and those to change last in the skill acquisition continuum. This analysis provides a method to facilitate person-centered care outcomes. For those with fewer resources, this model can be adapted to person-centered therapy in the home.

  Closed head trauma is one of the most commonly reported injury complaints in pediatric emergency departments and is a significant cause of pediatric death and disability worldwide. The Centers for Disease Control and Prevention (CDC) reports that among the 38 million youths who participate in organized sports in the United States concussion is the most common injury and has risen 57 percent among children (age 19 or younger). The events that lead to a traumatic brain injury (TBI) are usually predictable and preventable. The CDC wants to ensure the health and safety of our young athletes through their HEADS UP campaign initiative by informing athletes, parents and coaches about prevention, recognition and response to concussion.

  Providers can take an active stance to reduce and prevent brain injuries through educational efforts. Injury prevention education is one of the most effective approaches to decreasing the number of pediatric concussions. This webinar will address concussion truths and myths; tools for concussion identification, diagnosis and management; and concussion rehabilitation.

  According to the Defense and Veterans Brain Injury Center (DVBIC), 18,564 service members sustained a traumatic brain injury (TBI) in the first three quarters of 2014. The classification of mild TBI accounts for more than 80 percent of reported TBI to DVBIC. The vast majority of TBIs sustained are in a non-deployed setting in training exercises, motor vehicle accidents, and sports and leisure activities. These injuries directly affect the health and safety of individual service members and subsequently the level of unit readiness and troop retention.

  This educational forum will include a review of best practices in the assessment and treatment of TBI, the Military Health System (MHS) TBI Pathway of Care, key research studies and findings that will contribute to the body of TBI knowledge, and TBI prevention. We invite multidisciplinary health care providers in military treatment facilities and clinics, complex polytrauma facilities and community-based VA facilities to participate in this discussion of evidence-based practice and current research.

  Human behavior is a result of the constant interaction between the organism and its environment is a belief of the early 20th century psychologist Clark Hull (Freeman, Moore, & Freeman, 2009). Current research notes when injury or stress occurs, behavior reinforces the optimal biological conditions of survival.

  This is just as true today when a service member survives both the stress of injury from a brain insult and the effects of the concussion/TBI when either stress or TBI may be repetitive in highly paced or frequent deployment environments. When TBI occurs on a battlefield, both a TBI and a stress/fear response occur.

  If a threat or anxiety becomes overwhelming, the service member may revert to protective modes, including exacerbation of posttraumatic stress disorder, family conflict, alcohol or substance abuse, or other maladaptive reactions. Discussion will include a variety of evidence-based assessment and treatment strategies related to stress and anxiety following TBI.

• The Role of Sleep, Activity, and Nutrition in the Treatment and Recovery of Traumatic Brain Injury (TBI) (January)

  In 2013, Lt. Gen. Patricia D. Horoho, the U.S. Army surgeon general, launched an organizational initiative to improve the health, readiness, and resilience of the Army family. The performance triad is a comprehensive plan to promote the balance of sleep, activity, and nutrition among Army family members to improve health and wellness. The three key components of the performance triad represent sleep, activity, and nutrition that we understand to affect the cognitive and physical performance of service members.

  The performance triad can assist TBI patients in creating a system of healthy behaviors. The three components of the performance triad are instrumental in the recovery process for service members with a TBI, particularly those with persistent symptoms and chronic injury.

  Discussion will include the initial pathophysiology of brain injury, in relation to combat-related injuries and the physiology of stress relative to neural function. Evidence supports that chronic injury is the most complex of difficulties across the continuum of care from diagnosis to treatment and to recovery. Research studies indicate the integration of the performance triad of sleep, activity, and nutrition may serve to mitigate the effects of stress and even potentially promote positive neuroplasticity for recovery.

• Psychological Health Issues Affecting Women Service Members and Veterans (October)

  Women represent more than 15 percent of those serving in today’s U.S. military. Education and awareness of women’s health issues are key factors in maintaining current operational readiness. An increase in the presence and engagement of women in the military has resulted in an equally rapidly growing population of women veterans, projected to reach 15 percent of the total U.S. veteran population by 2035 (National Center for Veteran Analysis and Statistics, 2011). This presentation will describe relevant research findings and clinical guidelines to inform best practices for meeting the unique needs of this population.

  Current research findings indicate many women in the deployed environment hesitate to seek medical care when they have a health concern related to their genito-urinary or reproductive health. Issues around pregnancy and breastfeeding lead to occupational health concerns. Although important gender differences exist, similar to their male counterparts, women may develop mental health difficulties such as posttraumatic stress disorder and depression related to combat and/or military sexual assault.

• Suicide in Military and Veteran Populations: Implications for Chaplains, Health Care Providers and Leaders (September)

  Suicide is the 10th leading cause of death in the United States and accounts for nearly 40,000 deaths each year, according to the most recent statistics from Centers for Disease Control and Prevention. In the military and veteran populations individuals may seek care from chaplains and mental health professionals.

  This presentation will begin with a description of the epidemiology of suicide in the United States, with a specific focus on suicide among service members and veterans. An epidemiologic foundation of suicide provides context to understanding how current research relates to a variety of prevention strategies.

  The discussion will include the role and approach of chaplains to suicide prevention in the military. Using data from a survey of chaplains in the Army, this overview will review suicide prevention efforts and factors that may enhance or constrain effective support. • A Population Approach to Treatment Engagement in Behavioral Health Care (August)

  The webinar will address two major challenges relating to psychological treatment engagement in the military. First, many with psychological health conditions don’t access timely services. Second, most who access services drop out of treatment before it’s completed. Frontloading services in primary care can improve the first challenge. A recent Institute of Medicine report noted the growing burden of posttraumatic stress disorder among service members and veterans recommended an integrated, coordinated treatment strategy and measurement based care with feedback to clinicians to improve treatment engagement once patients access care.

  Webinar participants will review systems-level interventions that improve access and continuity of behavioral health care. Delivering high quality services in primary care can improve treatment access. Once in treatment, strategies such as motivational interviewing, behavioral activation and problem solving can increase continuity, help patients manage symptoms and improve functioning, adherence, outcomes and risk management. The addition of a care facilitator into staffing plans and workflows further strengthens continuity through improved engagement.

  Consequences of blast exposure can lead to a variety of ocular, otologic and vestibular injuries. Serious combat eye trauma accounts for approximately 15 percent of all battlefield injuries and up to 75 percent of those affected experience short or long-term visual dysfunction. Hearing loss and tinnitus prevalence for service members and veterans is increasing 13-18 percent annually and account for the top two most common service-connected disabilities among veterans.

  The majority of neurosensory disorders resulting from combat trauma are caused by mild traumatic brain injury. Understanding the mechanics of the injury and the associated research guides patient care and treatment recommendations. While there are assistive devices, technologies, training programs and support groups to assist those with vision or hearing loss, it is also important to provide psychological health support to the injured and their families.

  This webinar will examine current research and evidence-based practices to reduce injury risk and enable prevention and treatment in both clinical and non-clinical settings.

• The Limits of Adaptive Coping: Neurobiology of Extreme Stress (June)

  Despite unprecedented efforts over the course of the longest war in American history, existing programs in the Defense Department and Department of Veterans Affairs to prevent, identify and treat stress disorders among service members, veterans, and their families remain hampered by factors that limit their effectiveness. Research studies indicate that stigma, as a barrier to care, remains high in military communities. One limitation of existing psychological health programs, including those designed to reduce stigma, may be their exclusion of emerging scientific perspectives on the role of the central nervous system (CNS) in extreme stress.

  This webinar will examine the evidence that stress disorders are fundamentally neurobiological as well as psychosocial, and will provide an overview of select CNS neurotransmitter systems and neuronal pathways implicated in normal and pathological stress states. Discussion will include biologically informed approaches for psychoeducation to reduce stigma and for prevention and treatment in both clinical and non-clinical settings.

  The Diagnostic and Statistical Manual of Mental Disorders (DSM) provides a standard common language to classify mental disorders.   On Jan. 11, 1964, Surgeon General Dr. Luther Terry released the first Surgeon General’s Report on Smoking and Health. This scientifically rigorous federal government report not only linked smoking with ill health and diseases such as lung cancer and heart disease; it also laid the foundation for tobacco control efforts in the United States.

  Fifty years later, despite the release of 31 subsequent Surgeon General’s Reports on Smoking and Health detailing the devastating health and financial burdens caused by tobacco use, smoking remains the leading cause of preventable deaths in the United States and kills 443,000 people each year. (U.S. Department of Health and Human Services, 2014)

• Integration of a Virtual Hope Box Mobile Application into Clinical Care (October)

  Several behavioral health treatments have proven useful for patients who feel hopeless and may be considering suicide or self-harm. One tool used by providers is the “hope box”. Therapists suggest that their patients fill a shoe box (or other container) with items that remind them of why their life is worth living, such as supportive letters from loved ones, their favorite music CDs, photos, reminders of their accomplishments, or a list of things they want to do.

  However, since such a box is awkward to carry, it may not always be handy when a patient needs it most. Using the principles of the original hope box, a smartphone app was created called the “Virtual Hope Box” (VHB). As with its namesake, patients choose items, but the “container” is more portable and more private—and it’s always with them. Patients can also add more-accessible types of content like music files and video files.

  This webinar is designed to get the word out about Virtual Hope Box to clinicians, and show how smartphone apps can empower and engage patients in their own care.

  The Centers for Disease Control and Prevention reports that approximately 1.6-3.8 million sport and recreational concussions occur annually in the United States. Epidemiological studies conducted to understand sex differences between female and male athletes with respect to sport injury incidence and prevalence have shown that female athletes at both the collegiate and high school levels are at a greater risk for incurring a concussion. In addition, females have been found to report slightly longer concussion symptoms and cognitive impairments. The empirical findings on sex differences in sport-related concussion warrant clinical considerations when assessing concussion among male and female athletes.

  Reports of concussions during sporting events are increasing despite improvements in personal protective gear. Sports medicine experts recently published statements about the lack of effectiveness helmets and other equipment have on the incidence and severity of concussion. However, a recent study indicates that helmet design can reduce the transmission of impact forces to the brain and reduce concussion incidence.

  Biomechanical determinants of injury risk can be measured even though an objective marker of mild traumatic brain injury diagnosis is not available. Determining an acceptable level of risk, risk variation with age, directional variables and other factors is important. The effects of sub-concussive blows, multiple concussions and the interval between them are largely unknown. This webinar will address the biomechanical determinants of concussion risk and their relevance to real world sports injuries.

  Headache is one of the most common persisting symptoms after traumatic brain injury (TBI) across all levels of injury severity. • Family Caregiver Support after Traumatic Brain Injury (April)

  Family caregivers require and benefit from specialized support, education and understanding from health care professionals as they move through stages of adaptation that often correspond with the treatment and recovery of a service member who sustained a traumatic brain injury (TBI). In 2010, the Defense and Veterans Brain Injury Center, in collaboration with the Defense Health Board, developed “Traumatic Brain Injury: A Guide for Caregivers of Service Members and Veterans.” This guide contains four modules that provide in-depth information for family caregivers of service members or veterans with TBI. This includes information to help family caregivers understand TBI, how to advocate, help children cope with TBI and how to navigate services and benefits.

  This webinar will review common emotional and cognitive responses, caregiver responsibilities, learning needs and educational interventions. • Progressive Return to Activity Following Acute Concussion/Mild Traumatic Brain Injury: Guidance for the Primary Care Manager and Rehabilitation Provider (March)

  Consensus panels have increasingly recommended a gradual return to normal activity using a graded protocol after a concussion. In January 2014, Defense and Veterans Brain Injury Center (DVBIC) released two companion clinical recommendations to facilitate a standardized, staged return to unrestricted activity for service members who have sustained concussion in deployed and non-deployed settings.

  The “Progressive Return to Activity Following Acute Concussion/Mild Traumatic Brain Injury: Guidance for the Primary Care Manager and the Rehabilitation Provider in Deployed and Non-deployed Setting” resource suite was developed in collaboration with military, clinical and academic subject matter experts. It addresses the six stages of progression from rest to pre-injury activity; definition of rest; use of the Neurobehavioral Symptom Inventory to track symptoms; a two-tiered complementary approach based upon symptoms and provider judgment; physical, cognitive and vestibular/balance activities recommended for participation and activities to avoid at each stage; and guidelines for progression, regression and referral. This webinar will introduce these key stages delineated by the clinical recommendations.

  Traumatic brain injury (TBI) occurs when trauma to the head disrupts the function of the brain. TBI is commonly known as the signature wound of the Afghanistan and Iraq conflicts. Drawing on personal experience at the National Atlantic Treaty Organization Role 3 Hospital in Kandahar, Afghanistan, the presenters will share their approach to TBI management in the deployed, multidisciplinary setting. • State of the Science: Clinical, Metabolic and Pathologic Effects of Multiple Concussions (January)

  During the past few years, professional athletes and their families have become increasingly concerned about the long-term neurologic effects of repeated concussions. The premature deaths of several professional football players have drawn attention to the relationship of multiple concussions with neurocognitive disorders that occurred decades after these injuries, although similar changes in the brain were associated with boxing and first described in the 1920s.

  This webinar will examine the findings of laboratory and clinical studies on the mechanisms responsible for brain injury following one or multiple concussions. The presentation will address a particular focus on a class of proteins called Tau protein and the association of some of the histologic features of multiple concussions with those of Alzheimer's disease.
