Definition

An Operation & Maintenance (OM) file is opened when the final close-out report is accepted and is closed at deletion.  There is a long-term monitoring process following remediation and OF period, generally from five years to thirty years.  This file concerns activities at a site after a response action occurs, to ensure that the cleanup or containment system is functioning properly.  The state is usually responsible for monitoring the OM phase which can last for 30 years or more.  EPA hands responsibility to the state by a formal letter.

SDMS Keyword Description

25.01  Workplan / Amendments
A workplan outlines the approach for studies or activities as submitted by those responsible for undertaking the work for OM.  There is generally a draft submitted for review and comments, and then a final.  Following final approval of the workplan, amendments are used to change what was originally designated or agreed upon.  Workplans can include plans for Sampling Plans, the Health and Safety plan, QA/QC plans, etc.  If the workplan is for an EPA contractor and includes detailed cost information the entire workplan should be filed in Contractor Confidential (CC) [91.01].

25.02  Technical Progress Reports
These are routine progress or monitoring reports generated during the OM phase.

25.03  Five-Year Reviews
This is a report of the inspection conducted each five years during the OM phase to ensure that the site remains protective of human health and the environment.
 
25.04  Operation & Maintenance Reports
These are other reports or special studies generated during the OM phase.

25.05  Sampling and Analysis
These are any reports summarizing raw data related to any samples taken at the site and sent to a laboratory for analysis during the OM phase.

25.06  General Correspondence
Any correspondence including letters, memoranda, notes, and record of communications generated or received by the EPA.

25.99  Other
