MACAU, SPECIAL ADMINISTRATIVE REGION - *Passport required (valid for 6 months beyond the intended period of stay). Visa not required for visits of up to 30 days. MACEDONIA - *Passport required. Visa not required for tourist/business stay of up to 90 days. MADAGASCAR, REPUBLIC OF - *Passport and visa required. Visa allows entry to Madagascar within 6 months from the date of issue. Short-term visas valid for single-entry up to 90 days, $33.45; or double-entries, $39.01 (cash, certified checks, or money orders payable to Embassy of Madagascar only). Requires 1 application form, 1 photo, proof of onward/return transportation, and sufficient funds for stay. For tourist/business visa, submit the general requirements and include a recommendation from the travel agency or the company. Include a prepaid envelope for return of passport by registered mail. MALAWI - *Passport required. U.S. passport holders do not require a visa to enter Malawi. Thirty-day visa, which can be extended up to an additional 60 days, issued at point of entry. For longer stays, visa requires 2 application forms, 2 passport sized photos, passport with minimum of six months validity, travel itinerary, and an invitation letter. Visa fees (payable by money order or company checks): $70 single entry valid for 6 months; $110 multiple entry valid for 6 months; $150 multiple entry valid for 12 months. Transit visa is $50.There is a $30 airport departure tax (payable only in U.S. dollars). MALAYSIA - *Passport required (minimum 6 months validity remaining). Visa not required for stay of up to 3 months, extendable for 2 months. Medical exam required for work permits. U.S. test sometimes accepted. Foreign workers must undergo full medical checkup (HIV, hepatitis, DRL, drug abuse, and pregnancy) within one month of arrival and on a yearly basis. MALDIVES - * A valid passport, along with an onward/return ticket, proof of hotel reservations and sufficient funds, is required for entry. Tourist visa issued upon arrival for 30 days validity, no charge (for stays longer than 30 days, you must apply at the immigration office in Maldives). The Department of Immigration and Emigration routinely approves requests for extension of stays up to 90 days for travelers who present evidence of sufficient funds and who stay in a resort or hotel or present a letter from a local sponsor. Anyone staying over 60 days without proper authorization faces heavy fines and deportation. All visitors departing the Republic of the Maldives (except diplomats and certain exempted travelers) must pay an airport departure tax. MALI - *Passport and visa required. Obtain visa in advance. Tourist/business single-entry visa for stay of up to 3 months, extendable after arrival, requires $100 fee (cash or money order), 2 application forms, 2 photos, proof of onward/return transportation, proof of yellow fever vaccination, and the actual passport. For business travel, must have company letter stating purpose of trip. Send SASE for return of passport if applying by mail and/or appropriate express mail fees. MALTA - *Passport with minimum validity of six months required. Visa not required for stay of up to 90 days (extension must be applied for prior to end of 90-day period or expiration of original visa while in Malta), except for the residents of the Commonwealth of the Northern Mariana Islands. Residents of the Commonwealth of the Northern Mariana Islands need a visa. Visa requires 1 application form, 2 photos, proof of onward/return ticket, and $40 fee (check or money order). MAURITANIA - *Passport, visa, and proof of yellow fever vaccination required. Visa requires a valid passport, 2 signed completed application forms, 2 passport sized photographs. Tourist visa requires onward/return ticket and proof of sufficient funds. Visa fee $60 valid 3 months; $75 valid for 6 months (payable by money order or certified check only). MEXICO - *Passport, or proof of citizenship (such as original birth certificate or naturalization certificate) and photo ID. Tourist card is required. Tourist card valid 3 months for single entry up to 180 days, $20 fee, requires proof of U.S. citizenship, photo ID, and proof of sufficient funds. Visa not required of U.S. citizens for tourist/transit stay of up to 30 days. Obtain tourist cards in advance from Consulate, Tourism Office, and most airlines serving Mexico upon arrival. Departure tax $10 is paid at airport when not included in the cost of the airline ticket. Notarized consent from parent(s) required for children under age 18 traveling alone, with one parent, or in someone else’s custody. MICRONESIA, FEDERATED STATES OF (Chuuk, Kosrae, Pohnei, and Yap) - Passport (must be valid for at least 120 days beyond the date of departure), birth certificate or FSM entry permit. Sufficient funds and onward/return ticket required. Stays exceeding 30 days require an entry permit, extendible (up to 1 year from date of entry) after arrival in Micronesia; obtain forms from airline. Upon arrival without a valid entry permit, fill out and submit an FSM Immigration Arrival and Departure Record. Departure fee for Chuuk, Kosrae, and Pohnpei is $10 (U.S.), and $5 (U.S.) for Yap. Health certificate may be required if traveling from infected area. HIV test required if staying over 90 days; U.S. test is accepted. MIQUELON ISLAND - Passport required. Visa not required for stay of up to 1 month, and compulsory for stays between 1 and 3 months. Visa requires 1 application for short stay visa, valid passport and photocopy, onward/return ticket, proof of accommodation, proof of valid health/accident insurance and 1 copy, proof of sufficient funds (last 2 bank statements and 1 copy), and appropriate fee. Business travelers require a letter from employer guaranteeing travel expenses. Minors traveling alone must have notarized parental authorization signed by both parents or by non-accompanying parent. MOLDOVA - *Passport and visa required. Visa requires 1 application form, passport with a minimum of six months validity, 1 photo (1.5” x 2.0”), and processing fee. Visa fees: single-entry visa, $40; multiple-entry $50 to $200 (depending upon number of visits); transit visa, single entry $20; double entry $40. There is an additional $5 visa application fee that must be paid separately. All fees must be paid with company checks or money orders only. No visa fee for children under age 16. HIV test required for persons staying over 90 days; U.S. tests results sometimes accepted. MONGOLIA - *Passport required. Visa not required for private business and tourist stays of less than 90 days. Tourist and business travelers may be asked to show letter of invitation or a letter from a Mongolian company, and onward/return ticket. Obtain the approval of the Ministry of Foreign Affairs if applying for a multiple-entry visa or planning to stay for more than 90 days. Visitors planning to stay in Mongolia for more than 1 month are required to register with the Immigration, Naturalization and Foreign Citizens Agency within 7 days of arrival. American visitors who fail to register and stay longer than 30 days may be stopped at departure, denied exit, and fined. MOROCCO - *Valid passport required. Visa not required for stays up to 3 months. For visits of more than 90 days, apply for an extension (with a valid reason for the extension of stay). Travelers who plan to reside in Morocco must obtain a residence permit. A residence permit may be requested and obtained from immigration authorities at the central police station of the district of residence. MOZAMBIQUE - *Passport and visa required. Visa must be obtained in advance or tourist visa can be obtained in some borders in Mozambique. Multiple-entry visas valid 3 months from date of issuance, and single- and double-entry visas valid for 2 months. General visa requirements: 1 application form, 2 photos, a letter (from company or individual) giving detailed itinerary and address of where visitor will be staying, and payment of corresponding fees ($20 single entry, $40 double- or multiple-entry) as well as processing fee for 2 weeks or an additional $20 fee for 3-day service and additional $50 fee for same-day service.
