US Family Health Plan

The US Family Health Plan is an additional TRICARE Prime option available through networks of community-based, not-for-profit health care systems in six areas of the United States.  It's the only TRICARE Prime program that offers benefits to beneficiaries age 65 and over, regardless of whether or not they have Medicare Part B.

You may enroll in US Family Health Plan at any time during the year by completing an application for the provider (or plan) in your area.  Select your US Family Health Plan from the list below to download the correct enrollment application:
