Disenrolling


Enrollment in the US Family Health Plan is continuous. You do not have to re-enroll every year to continue coverage, but certain events will cause you to be disenrolled.

Voluntary Disenrollment
If you choose to disenroll from the US Family Health Plan before the annual enrollment renewal date, you are subject to a 12-month lockout.  For 12 months, it is possible that you may be prohibited from re-enrolling in the plan. You must contact your US Family Health Plan provider to initiate a voluntary disenrollment.

Loss of Eligibility
If you lose eligibility for TRICARE, your US Family Health Plan coverage will automatically end. You will receive a
certificate of creditable coverage when TRICARE eligibility is lost.  The certificate of creditable coverage serves as proof of insurance so you cannot be excluded from a new health plan for pre-existing conditions.  

Upon loss of eligibility, you will qualify for transitional health care benefits under the Continued Health Care Benefit Program.
