Example: Tailor existing patient education materials to your needs (NDEP materials are public domain), and ask community partners to donate printing, artwork, even storage space

  • Examples of Partnerships with Special Populations
    There are additional needs, considerations and opportunities for special populations. There may be other key partnership links. For example, the NDEP Community Partnership Guide American Indian and Alaska Native Supplement suggests partnerships with Tribal Health Coordinators, Tribal Leaders, American Indian organizations, Indian Education Program directors.

    Community partnerships are key for providing appropriate, consistent and effective care for homeless and migrant patients. The Health Disparities Collaborative website www.healthdisparities.net describes two resources for these populations.

  • Examples of Successful Partnerships
    The Task Force on Community Preventive Services is a 15 member non-Federal task force supported by the Centers for Disease Control and Prevention (CDC). To improve the health of people with diabetes, the Task Force reviewed the evidence of the effectiveness of diabetes disease and case management and self-management education. The Guide to Community Preventive Services reviews interventions that occur outside traditional clinical settings, such as the home and faith institutions. Details of this review and the final recommendations are available at www.cdc.gov/mmwr/preview/mmwrhtml/rr5016a1.htm (MMWR Sept 28, 2001/Vol 50/No RR-16) "Strategies for Reducing Morbidity and Mortality from Diabetes Through Health-Care System Interventions and Diabetes Self-Management Education in Community Settings."
