National Standards for Diabetes Self-Management Education (DSME) Programs care.diabetesjournals.org/cgi/content/extract/31

The National Standards for DSME are designed to define quality diabetes self-management education that can be implemented in diverse settings and will facilitate improvement in health care outcomes. They are designed to be flexible enough to be applicable in any health care setting, from physicians' offices and HMOs to clinics and hospitals.


Education Recognition Program

The American Diabetes Association Education Recognition Program assesses whether applicants meet the National Standards. The website provides a number of sample documents to help applicants meet the requirements including:

  • Mission statement
  • Behavior change goals and follow-up
  • Patient information form
  • Gestational education assessment and record
  • Physician prescription form.


Integrated Diabetes Education Recognition Program

The Indian Health Service offers an Integrated Diabetes Education Recognition Program based on implementation of the National Standards for DSME and provides sample materials for developing quality diabetes education programs.

Diabetes Physician Recognition Program

The Diabetes Physician Recognition Program, cosponsored by National Committee for Quality Assurance and the American Diabetes Association, assesses physicians on their performance on measures of care for adults with diabetes and for children with diabetes. The goal is to help prevent the complications of diabetes, encourages the provision of comprehensive, quality health care to people with diabetes.

 

National Certification Board for Diabetes Educators (NCBDE)

The NCBDE credentials an individual as a Certified Diabetes Educator (CDE). The credential demonstrates that the certified health care professional possesses distinct and specialized knowledge, thereby promoting quality care for persons with diabetes.

 

Certification for Advanced Clinical Practitioners Specializing in Diabetes Management

American Association of Diabetes Educators offers this certification of advanced practice to signify attainment of specific criteria, knowledge, skills, and abilities in diabetes care. It enhances opportunities for a more focused interdisciplinary approach to diabetes management.

These lesson plans serve a variety of health education activities, such as a lecture series, a diabetes support group curriculum, and "Lunch and Learn" forums.

  • Outline with objectives, script guide, suggested group activities or exercises, and recommendations for shortening the lesson to a 15- to 20-minute time period
  • Handouts
  • Overheads
  • PowerPoint slide sets.
