Diabetes Care Profile
A self-administered questionnaire that assesses the social and psychological factors related to diabetes and its treatment.

Diabetes History
A form that has been used by the MDRTC for the past 15 years to collect basic clinical diabetes information from community-based patients involved in a variety of MDRTC projects. The survey instrument is designed to be self-administered.

Diabetes Knowledge Test
A test consisting of 23 knowledge test items developed by MDRTC and used as a test of general knowledge of diabetes.

Diabetes Attitude Scale (DAS-3)
A scale that can be used with both people with diabetes and health care professionals as a measure of general diabetes related attitudes.

Diabetes Empowerment Scale
A 28 item Likert-type questionnaire with three subscales: managing psychosocial issues, assessing dissatisfaction and readiness to change, and setting and achieving goals. The scale is a valid and reliable measure outcome measure for a variety of educational and psychosocial self-efficacy interventions related to diabetes. It is labeled Diabetes Attitude Questionnaire for people with diabetes.

Michigan Neuropathy Screening Instrument
An instrument designed to screen for the presence of diabetic neuropathy. It includes both a self- assessment of symptoms and a brief physical examination. It is for use in an outpatient setting by primary care or other providers.
