Determining Needs and Setting Priorities for Systems Change

Overview

Making a systems change is a lengthy and important process, involving many steps, people, resources and tools. In order to complete the transition in an effective and efficient manner, a solid foundation of data, needs assessment, models and goals is needed. This section covers "places to begin" for organizations ready to start assessing needs and determining priorities.

This section:

• Defines microsystems.
• Reviews reasons for self-assessment for systems change and how systems change can benefit both practices and organizations.
• Presents IOM recommendations for systems redesign.
• Provides three tools used in a variety of settings as examples of practical assessment tools that can guide quality improvement efforts and evaluate changes in diabetes care.
