LAOS - *Passport with minimum of six months validity and visa required. Pay the $35 fee and $15 service charge (payable by money order or certified check only), and submit 2 application forms, 3 recent photos, onward/return ticket, and sufficient funds. Tourist visas can be obtained upon arrival at certain entry points or before departure from the Lao Embassy. Visa available for a stay of up to 15 days, extendable. LATVIA - *Passport with minimum validity of 3 months required. Visa not required for a stay of up to 90 days. For longer stays, visa requires 1 application form (minors’ application forms must be signed by one parent). Written consent of the parent/guardian having custody of the child must be included. To work, study, or live in Latvia a temporary work permit must be obtained. A residence permit must be requested through the embassy. Valid health insurance policy (policy should cover all costs relating to medical care during stay) required for all visitors. LEBANON - *Passport and visa required. U.S. citizens staying no longer than 1 month (extendable up to 3 months) can obtain their visa upon arrival in Lebanon after paying the $35 fee. For longer stays, visa must be obtained in advance from the Lebanese Embassy or Consulate. Visa requires 1 application form, 2 passport sized photos, a valid passport, photocopies of passport pages containing the full name, photo, passport number, issue date, and expiration date. For minors, a parental consent form from both parents is required. Fee for single-entry visa is $35 and $70 for a multiple-entry visa. Fee must be paid by money order payable to the Embassy of Lebanon. Business visa requires company letter stating purpose of travel. HIV test required for those seeking residence permits; U.S. test is accepted. Minors need authorization from both parents (a notarized parental consent form/letter) to enter Lebanon. Note: All visa applicants holding an Israeli visa on their passport, at the time of applying for visa or arrival in Lebanon will not be granted a visa and/or be admitted to Lebanon without the prior approval of the Lebanese Immigration Authorities. LIBERIA - *Passport, visa, and evidence of a yellow fever vaccination required. Travelers must obtain visas before arrival. There is a $100 (U.S.) airport tax on departing passengers. Visa requires 2 application forms, 1 valid passport, 2 passport photos, proof of sufficient funds, letter of request, and proof of place of accommodation. Visa fees: $100, single entry valid 1-3 months; $200, multiple entry valid 1 year; $300, multiple entry valid 2 years; $400, multiple entry valid 3 years. LIBYA - *Passport and visa required. Restrictions on the use of a U.S. passport for travel to, in, or through Libya have been lifted as of February 2004. At this time, neither Libya nor the U.S. provides visa services in each other’s countries; U.S. visitors to Libya should therefore plan to obtain a visa via a third country. Visa applications and inquiries may be made through the Libyan Mission to the United Nations in New York. Within 3 days of arrival, visitors must register at the police station nearest where they will be residing. Attention: U.S. citizens need a U.S. Treasury Department license in order to engage in any transactions related to travel to and within Libya. LITHUANIA - *Passport required. Visa not required for tourist/business stay of up to 90 days. For longer stays, (e.g. employment, research, study, training) obtain temporary residence permit before arrival (processing time of the application for the first temporary residence permit is up to 6 months). Visitors may be subject to HIV testing to obtain residence permit (U.S. results accepted). All visitors to Lithuania must have a valid health insurance policy or contract that covers all health expenses during their stay. Some countries require that your U.S. passport be valid at least 6 months or longer beyond the dates of your trip. If your passport expires before the required validity, you will have to apply for a new one.
