AIR FORCE For Periodic Health Assessments (PHAs), complete the self assessment through the link below, print out your assessment and make an appointment at the clinic. Ensure you inform the front desk that you are making an appointment for a PHA and ensure you bring a copy of your assessment print out with you. Over 40 years old? If yes, labs will be ordered for you during you initial visit. You will need to fast for 10-12 hours before returning to the clinic at your leisure for your labs to be drawn. You may take your prescribed medication with water and brush your teeth but you may not eat, drink other fluids, chew gum or take breath mints. If you have fasted prior to your initial appointment then the labs can be drawn immediately following the appointment. You will then need to make another appointment for part 2 of the Air Force over 40 PHA no sooner that 2 weeks after your labs are drawn in order to ensure the results have been recieved. You will receive a copy of the completed PHA before you leave the clinic. You must hand carry this to your unit readiness office. For deployment assessments (pre [within 30 days before deployment], post [within 30 days post-deployment], and post reassessment [90-180 days post-deployment]) fill out the assessment form and stop at the "health provider" section. Make an appointment at the clinic. Ensure you inform the front desk that you are making an appointment for a deployment assessment. You will receive a copy of the completed deployment assessment before you leave the clinic. You must hand carry this to your unit readiness office.
