Preseparation counseling represents more than a completed DD Form 2648 or 2648-1. The preseparation counseling experience introduces you to your benefits and helps you gain more from the time you spend with individual service providers. While it does not answer all your questions, preseparation counseling does direct you to knowledgeable installation service providers and comprehensive online resources where you can find answers. You can use this website to schedule preseparation counseling at an ACAP Center or receive counseling and complete DD Form 2648 or 2648-1 online. Whichever method you choose, during preseparation counseling, you will listen to brief descriptions of separation benefits and mark items to indicate you want to learn more about a benefit before you leave active duty. In this way, your completed DD Form 2648 or 2648-1 can serve as a roadmap that will guide you to resources and a tool that can help you find answers for any questions you have.

Your completed Checklist will not contain all the detailed information you may need, so as part of your transition process you will participate in briefings with many of the service providers shown on the Checklist. You will hear a lot of valuable information, but you may not remember everything you hear. Once again, your Checklist will help organize your transition by identifying Active or Reserve Component benefits and services and linking you to websites or providers where you can learn more about selected items. Once completed, your DD Form 2648 or 2648-1 will become part of your permanent personnel record. A copy of your completed Checklist must be part of your record before you can out process.
