Definition

An AS file is opened when a State requests the first agreement and is closed upon site deletion. This phase contains agreements [Cooperative Agreement (CAG) or the State Superfund Contract (SSC)] or Memorandum of Understanding (MOU) between EPA and the State, or the State and other entities, concerning Superfund site clean-up and/or cost sharing issues. State agreements concerning Superfund clean up and related correspondence, whether with EPA or not, are filed in this phase. Technical reports are filed in the applicable phase.

15.01 Cooperative Agreements
This is an agreement between EPA and a state agency arranging for cooperation in cleaning up a site. See also [15.06] State Superfund Contract.

15.02 Cooperative Agreement Amendments
These are changes or amendments to the agreement between EPA and any state agency.

15.03 Quarterly / Monthly Budget Reports
These reports from the state are submitted quarterly or more often work progress, and accounting for money expended on the site. Note: Budget reports and technical progress reports can be combined in one document.

15.04 Cooperative Agreement Technical Progress Report
This is a technical report received from the state on a monthly or quarterly basis concerning the work progress at the site.

15.05 Other Cooperative Agreement Deliverable
These are any other reports or documentation required under the cooperative agreement from the state which could include workplans such as a Quality Assurance Project Plan or a Health and Safety Plan.

15.06 State Superfund Contract
This is an agreement between EPA and the state documenting responsibilities and obtaining the necessary state assurances including cost-sharing.

15.07 State Superfund Contract Deliverable
These are any reports or other documentation required by the State Superfund Contract from the state.

15.08 Multiple Site Cooperative Agreement
This is an "umbrella" cooperative agreement that provides one funding document for several response activities at more than one site, often for removal action at several sites in close proximity.

15.09 State / PRP Agreement

15.10 General Correspondence (Removal / Remedial)
Any correspondence including letters, memoranda, notes, and record of communications generated or received by the EPA.
