Tool Kit
The Texas Diabetes Council’s Diabetes Tool Kit
was prepared by an interdisciplinary team of volunteer certified diabetes
educators (CDEs) and professional staff of the Texas Department of State
Health Services Diabetes Control Program to be of service to Texas practitioners
and diabetes educators working with patients who have diabetes. Many partners
contributed to its development, revisions, and distribution.
Print versions of the Tool Kit can be ordered
using an online order form. Please note that Tool Kits are no longer available
for order through the Texas Department of State Health Services Forms
and Literature Inquiry and Order Entry System. Order options include:
- a print format Tool Kit with binder (limit 5),
- updated contents pages to replace older versions of the Tool Kit you
already have (print format Tool Kit without binder - limit 5), or
- a CD format containing the Tool Kit as PDF files (limit 10).
Option 2 is recommended for those who already have a Tool Kit with a
binder in good condition. An updated cover and contents pages is provided
to update your existing Tool Kit.

The Diabetes Tool Kit Features:
(1) Self-management training content based on the National Standards
for Diabetes Education; (2) Minimum Standards of Care and evidence-based
treatment algorithms prepared by volunteer endocrinologists, physicians,
nurses, dietitians, pharmacists, and other professionals on the Medical
Professionals Advisory Subcommittee of the Texas Diabetes Council.
The Diabetes Tool Kit is a resource that includes professional and patient
education materials. The Kit assists primary care providers, educators,
and health plans to deliver quality care and to implement quality improvement
efforts.
Basic copy masters in English and Spanish help primary care providers
and educators address basic
self-management education with their patients who have diabetes. These
tools assist those who conduct diabetes self-management education, case
management, or disease management.

The Fourth Edition of the Diabetes Tool Kit is divided into the three
sections indicated below. Open or download each PDF file and use the table
of contents at or near the beginning of each document to navigate to topics
