The National Diabetes Education Program has developed a number of unique education materials that address the needs of diverse populations. Publications are copyright-free and may be duplicated and distributed. You can find the materials in a variety of formats for physicians and other health care professionals including pharmacists, podiatrists, optometrists, and dental professionals that address diabetes prevention, foot care, numbers at a glance, guiding principles, pediatric care, diabetes medications, and team care. Many materials for patients are also available.

The AHIP Compendium and Toolkit were developed for the first annual Taking on Diabetes: Care in the New Millennium conference, August 3-5, 1999 in Washington, DC. The conference, sponsored by the American Association of Health Plans (now AHIP), the Health Care Financing Administration (now Centers for Medicare and Medicaid Services or CMS), and the American Diabetes Association (ADA), focused on three elements of successful diabetes care: medical management, worksite health programs, and community partnerships.

  • Compendium. The Compendium includes peer-reviewed abstracts of published studies demonstrating improvement in diabetes health care, peer review organization national projects on diabetes, and community and worksite projects.
  • Toolkit. The toolkit contains outreach tools targeting patients, providers, and the community, as well as organizational flow sheets and charts designed to assist providers and health plans in the provision of high-quality diabetes care. The tools can be utilized as is or can be adapted or modified to meet an organization's needs.
