Assist in establishing an integrated surveillance system, Sistema Integrado de Vigilancia de las Intervenciones de Nutricion (SIVIN) to monitor the impact and coverage of nutritional interventions in Nicaragua

Background and Purpose
The CDC and the USAID established an interagency agreement in fall 2000 to work jointly on international micronutrient related issues. To support specific country projects, CDC provides funds and technical assistance for MOST, the USAID micronutrient project. Support for SIVIN in Nicaragua is one of the projects supported through the CDC-USAID collaboration. While SIVIN provides information for the nutritional interventions in Nicaragua, IMMPaCt assistance focuses on the micronutrient interventions.

Levels of coverage and impact vary with the type of intervention. Assessment of biological impact has previously been conducted (1993�00) through periodic national micronutrient surveys.
The objective of the project in Nicaragua is to work with the government to implement a sustainable system to monitor household coverage and biological impact of micronutrient interventions. This monitoring system will also indicate possible changes in breastfeeding practices. Anthropometric data will also be obtained for participating women and children to help monitor overall nutritional status. Information from this monitoring system will be used to evaluate the effectiveness of these nutrition interventions and to change programs as necessary.
A small team, established in the Ministry of Health will collect data in predetermined areas of the country every 4 weeks. The team will also implement the surveillance system. The data will provide national prevalence estimates of micronutrient deficiencies in women of reproductive age and children under 5 years old on an annual basis. Regional estimates for the three most populous regions will be available for every three-year period.

Progress and Future Plans
- Multiple planning missions to Nicaragua have been conducted.
- High level support has been obtained from the National Micronutrient Commission and the Nicaraguan Ministry of Health.
- Technical assistance was provided by CDC to develop the survey design for the surveillance system and all field and data management/analysis tools, to train the national team, and to implement the first round of surveillance in July 2002.
- A three- month review will be conducted in October 2002.
- The Nicaraguan government is expected to gradually take over full technical and financial responsibility for the program by the year 2005.
