Travel Expense Form

Who should use this form?

Only the lead presenter or exhibitor, Career Development awardees and mentors attending the CDA conference, meeting planning committee members, panelists, invited guests, or meeting staff members should submit this travel form.

  • Download the Travel Expense Form (MSWord, 57K)
NOTE: Save this form to your computer before filling it out.

For VA Participants Only

All VA travelers must reduce their per diem for meals provided at the meeting — breakfast and lunch on 2/12 and breakfast on 2/13. For those attending the CDA conference, breakfast and lunch will be provided on 2/11. The meeting fee will be included as part of your travel reimbursement, if your travel expenses are being reimbursed by HSR&D Central Office.

Travel estimates from individual VA travelers will not be processed without approval from local Administrative Officers or Budget Officials. Your Administrative Officer or Budget Official must approve your travel estimate and return the form via fax or they may indicate their approval via email to Rama Palriwala (see instructions on form). This is to help reduce the rate of corrections and provide for better travel estimates.
