Passport Information
Guidelines for obtaining your Passports can be obtained from my office.
Please contact my office with any questions you may have about this process.
And, most importantly, the process of obtaining a passport can sometimes
over six weeks. Please plan your travel plans accordingly.
OBTAINING YOUR FIRST U.S. PASSPORT:
Complete Passport Application Form DS-11 in person at an acceptance facility
which could include your local Clerk of Courts/Prothonotary's Office,
U.S. Postal Service or a local travel agency (click
here to find out where you can apply). PASSPORT APPLICATIONS MUST
BE SIGNED IN THE PRESENCE OF THE PASSPORT ACCEPTANCE AGENT. Applicants
under 14 years of age usually do not need to appear in person; a parent
or guardian may execute the application on the child's behalf. Children
ages 14 to 17 must appear in person to execute the application, and should
be accompanied by parents.
APPLICANTS WILL NEED TO PROVIDE THE FOLLOWING WITH THEIR APPLICATION:
- Proof of Citizenship (US born - certified birth certificate with raised
seal issued by bureau of vital statistics. Foreign born - certificate
of naturalization or citizenship; consular report of birth/FS-240; certification
of report of birth abroad/DS-1350)
- Identification (the following are generally acceptable if they readily
identify the applicant and/or the parent) A valid US driver's license;
naturalization certificate; government employee i.d. card; official
military i.d. card; or previous US Passport
- Photographs - You will need to present 2 identical passport photographs
taken within the past 6 months, 2 x 2 inches in size. They must be front
view with a white or off white plain background.
Passport Applications can be obtained at any acceptance facility, or
downloaded at http://travel.state.gov.

PASSPORT RENEWALS:
You may be eligible to use Passport Application Form DS-82 (mail-in
application).
PASSPORT FEES:
- $97 if you are 16 years of age or older ($55 passport fee/$30 application
execution fee/$12 security surcharge).
- $82 if you are under 16 years of age ($40 passport fee/$30 application
execution fee/$12 security surcharge).
EXPEDITED PASSPORTS:
Should you need your passport in a hurry, the cost is $60 per
application IN ADDITION TO all regular application fees AND overnight
delivery costs. You will recieve your passport in about 2 weeks through
this procedure.
OBTAINING A PASSPORT IN AN EMERGENCY:
If you are in an urgent situation and have to depart in 5 days or less,
you may contact the Philadelphia Passport Office directly by calling (877)
487-2778. This is the constituent line for making status check inquiries
and for urgent matters such as making an appointment to travel to Philadelphia
in order to obtain same day service.
