The report forms listed below are required to be submitted at various times during the project. For information regarding the frequency of these reports, refer to "Managing Your Grant."

These forms are in PDF format. To ensure you will be able to see a PDF file in its entirety, please obtain the most recent edition of the free Acrobat Reader from the Adobe Web site.

The forms are available for download and use. Note that they are also 'writeable'. You may fill in the forms while you are viewing them and print the completed form. You may also save them to your local computer for future use.

If you have the full Adobe Acrobat software package, you will be able to save a copy of the form with your input. If you have downloaded only a copy of Acrobat Reader, you will be unable to save your input with the form.

IMPORTANT UPDATE FOR ALL EPA REGION III GRANT RECIPIENTS

Recently, the Office of Management and Budget (OMB) approved the new Federal Financial Report (FFR), Standard Form (SF) 425 for use throughout the Federal Government. The FFR was designed to replace the Financial Status Report (SF269) and the Federal Cash Transactions Report (SF272) with one comprehensive financial reporting form.

By October 1, 2009 each Federal Agency must issue instructions and fully adopt the SF425 form for use by their respective recipients.

Until the implementing instructions for transition to this new Form are developed and issued, Region III requests that all grant recipients continue to use the SF269 and SF272 forms in accordance with the terms and conditions of their assistance agreements. Please do not use the new SF425 FFR Form until the EPA specific instructions are made available to you.
