 Original Equipment Manufacturer (OEM) Load-locking Valves
    - Dated October, 2008
    Loading and continuous-mining machines, since 1981, are required by MSHA to be equipped with load-locking valves in the boom and head lift cylinders. The load locking system installed must be maintained in proper working order.

 MSA Dyna-Lock® Self-Retracting Lanyard 20ft. Models
    - Dated September 4, 2008
    MSA recently received a report that the attachment handle on a 20 ft. Dyna-Lock Self-Retracting Lanyard (SRL) became loose and separated from the unit. Although the user was not injured, their inspection of the unit revealed that the ends of the two screws securing the handle were not swaged (flattened) as intended...

 ANSUL Fire Protection Products - Dated: 03/07/2007
    ANSUL® recently issued General Bulletin NO. 5208; regarding an important technical update for the Checkfire® SC-N detection and control module. This bulletin was issued due to the SC-N control module activating for undetermined reasons on surface mining equipment. Resulting investigation led ANSUL to determine that the activation was due to RF/EMI (Radio Frequency/Electro Magnetic Interference). A new component board was added to the Checkfire® SC-N detection and control module to decrease susceptibility to outside interference. Any operation experiencing activation with no apparent reason, please contact your ANSUL representative.

 Automatic Emergency Parking Brakes (AEPB) on
Long Airdox 400 and 600 Series Scoops
- Dated: 06/04/2008
    A scoop was recently cited for non-compliance of 30CFR 75.523-3(b)(2). If the machine's hand pump was used to release the AEPB when the scoop was off, when the scoop was re-energized, the AEPB would not reset until one of the hydraulic functions was operated. The AEPB was restored to normal operation when a hydraulic function was operated.

 Brookville Battery Powered Man Trips and Personnel Carriers with Hydraulic Brakes Dated: 04/2006
    Instructions for Properly Maintaining the Braking System


    In October of 2005, a non-fatal accident occurred when a personnel carrier lost its brakes, rolled down a grade, and collided with another vehicle. The machine involved in the accident was a battery powered Brookville Model ULPC, 13 Man Ultra Low Personnel Carrier. The personnel carrier’s braking system consisted of an electrical regenerative brake, a hydraulic service brake, and a spring applied parking/emergency brake. The service brake system failed at the time of the accident and the operator was unable to regain control of the machine using the regenerative or the emergency brake. Tests of the service brake system showed intermittent failure of the hand operated hydraulic service brake actuator (master cylinder). The investigation determined that debris found inside the actuator caused seal failure in the actuator.

 Caterpillar Incorporated Off-Highway Truck and Tractor Field Service Notification Dated: 01/2006
    MSHA has been notified by Caterpillar of a potential corrosion issue related to certain Caterpillar off-highway trucks and tractors. Specifically, engine exhaust may contribute to frame corrosion of the cab support structure. Caterpillar has launched an inspection program with provisions for repair, if required, that will affect a number of trucks and tractors in use at coal and metal and nonmetal mines throughout the United States. Please contact your local Caterpillar dealer immediately to schedule a service appointment. For specific information, please use the link provided below.

 CPSC, MAKITA U.S.A. INC. ANNOUNCE RECALL OF CHAIN SAWS
    Date: September 12, 2005
    Abstract: The recall involves Makita DCS6401 and DCS7901-model chain saws in which the last five digits of the serial numbers are within the following ranges: Chain saw Model Serial Number Range DCS6401 41915 through 45612 81722 through 82057 DCS7901 31182 through 31491 The chain saw's housing is teal with the name "Makita" written on it. The chain saw's serial number can be found on the right hand side of the silver nameplate which is located on the back of the fuel tank. No other Makita chain saws are involved in the recall. Any DCS6401 or DCS7901 chain saws with the letter "N" preceding the serial number on the nameplate and a blue dot on the shipping carton have been repaired and are not involved in the recall.

 CPSC, Robert Bosch Tool Corp. Announce Recall of Circular Saws
 Failure to Detect Loss of Brake Accumulator Pressure - Dated: 09/2007
    Recently in Australia, a loader traveling downhill lost its brakes after the engine stalled, causing the operator to lose control. The loader rolled over onto the operator who was crushed between the cab and the ground. It was found that the brake accumulator pressure was too low to provide effective braking, and several of the parking brake components were missing.

 Getman Service Bulletin Issued Dated: 04/2004
    PURPOSE: Fire Protection
    VEHICLES INVOLVED: Getman vehicles utilizing exhaust wrap

      It has come to the attention of Getman Corporation, that over a period of time, the exhaust wrap utilized to shield the exhaust systems of many Getman vehicles may deteriorate to the point of becoming non-functional. This deterioration can often be attributed to high-pressure washing, accidental damage and maintenance work in the general area.

    Model 4CF Carbon Monoxide Sensor


    Industrial Scientific has been notified by our supplier, City Technology, of an industry-wide recall of the model 4CF carbon monoxide sensor. A failure mode has been identified in this sensor that may exhibit itself as an unexpected, catastrophic loss of sensitivity, inhibiting the sensor’s ability to detect and respond to carbon monoxide.

 Insulation of Cable Reel and Spooling Devices - Dated: 03/17/2008
    The Mine Safety and Health Administration (MSHA) has found that an exposed, accessible cable roller(s) located between the sheave wheels and the area housing the cable reel and spooling device on a shuttle car can pose a shock hazard. The potential shock hazard is created when accessible, uninsulated and ungrounded metal roller(s) comes in contact with an exposed trailing cable. Rollers are typically located between the sheave wheel carrier and the cable reel housing/spooling device assembly.

    FLETCHER MACHINES THAT HAVE THE TEACH/LEARN RADIO TRASMITTER ONLY (PART NO. 121274)


    J.H. Fletcher & Co.(“Fletcher”) has been notified that an error in programming allows two separate radio remote machines to work from the same transmitter. This phenomenon could result in an unintentional movement of a roof bolter. This situation is limited to less than thirty (30) CHDDR machines.

    Owners Of Model DDO/LTDO Roof Bolters


    On January 31, 2005 a roof bolter operator was injured at a mine in West Virginia. The Mine Safety and Health Administration (MSHA) is still conducting its investigation.

    Preliminary information obtained by J.H. Fletcher & Co. (Fletcher) indicates that the operator using a Fletcher DDO roof-bolting machine was injured when the boom fell and the operator had his leg under the boom area. It is reported that the boom fell because the cap that holds the feed cylinder pin in place had broken loose.

    Fletcher is sending out this bulletin to remind all owners and operators of specific arm feed machines (DDO/LTDO roof bolters only) of the previously published warnings about the open and obvious pinch point dangers inherent in a roof bolting machine; the essential requirement that an operator never position any portion of his/her body in the pinch point area; the possibility of severe injury or death that could occur if these warnings are ignored; the importance of using only OEM components and proper maintenance of all equipment.

  • Joy has recently been made aware of an incident in which it was reported that when a Joy Lightweight Remote Control Station was placed on top of a magnet on a continuous miner a function occurred that was not intended. According to the report when the remote station was removed from the magnet this function then ceased. - See Alert
 Large Electrical Equipment at Surface Mines May Have Auxiliary Power Sources Dated: 11/2006
    COAL MINE FATALITY - An electrician, with 21 years of experience, was fatally injured while troubleshooting a dragline trailing cable at a surface coal mine. The electrician was working at an electrical junction box that supplied power to the dragline when he contacted two energized phase conductors. Incoming power to the junction box had been visually disconnected and locked out by the electrician. An onboard diesel powered generator was started and back fed 480 volts through on-board transformers connected to the dragline's trailing cable and energized the phase conductors to 23,000 volts.

 Manitowoc Crane Care - Dated July 25, 2008


    The crane division of manufacturer Manitowoc Co. Inc. has told operators of one of its models to shut the cranes down because of problems the company says could affect their structural integrity. The service bulletin, dated July 25, said there were problems with the width of chords that make up part of the lattice-like structure of the arms on Manitowoc's model 16000 series of cranes. It listed the serial numbers of 18 cranes, but did not say where they were in use.

    North Safety has identified certain conditions where Saf-T-Lok Sleeves or Rail may not perform properly during a fall. Under these conditions, a climber may fall farther than intended, resulting in serious injuries or death.

 Park/Emergency Brakes on Battery Powered Machines - Dated: 04/18/2008
    On March 11, 2008, Sandvik issued a Safety Bulletin, Park/Emergency Brake Retrofit Kit and Test Procedure for Battery Powered Machines. The safety bulletin was issued due to a MICO park/emergency brake found in a dry condition in an Eimco Coal Hauler. The park/emergency brake was designed to be used wet with hydraulic oil as the coolant. The safety bulletin affects all 508CS, 518CS, and 580CS battery powered machines and all MICO park brakes supplied under part number 3210166 by EIMCO, EJC or Sandvik (MICO model number 13-547-420). The safety bulletin can be accessed below. Please contact your local Sandvik representative for the field retrofit kit and/or more information and questions.
