Medical Surveillance/Examination Status

Ordinarily, medical surveillance is provided in connection with the routine recheck examination scheduled at OMC on a regular basis.

The OMC Staff identifies items which require specific medical protocols based upon information provided by the supervisor on the Job Assessment Form (JAF). Since job conditions or requirements may change from one examination to the next, an updated JAF is required prior to each recheck examination.

Certain specific types of medical surveillance depend upon workplace exposures and may not be timed appropriately if done at the time of the recheck examination. Therefore items not on the JAF  require specific triggers for OMC to provide medical surveillance. 

To request surveillance for items that are not on the JAF (for example noise, asbestos, or lead), complete the information on the Additional Medical Surveillance Form (.doc), and send the employee to OMC with the form.  This form may also be used in between examinations for those same items. No appointment is necessary.

Training Database

Dates for the most recent examination at OMC and completion dates for medical surveillance programs may be found on the BNL Training Database website. This website lists completion dates of the last examination or surveillance. 

To view a record:

  1. Click on http://training.bnl.gov/
  2. Click on Training Record.
  3. Click on OMC Medical Surveillance/Physical Information.
  4. Type in the last name or life number of the person whose record you want to view.
  5. Click Submit.

Note: Examinations and medical surveillance information on the training database are prefixed by OM.  These are not the same as training requirements.
