IANOV 04/30/2007 On April 30, 2007, a Notice of Violation was issued for a Severity Level III violation involving the deliberate submittal of information that the individual knew to be inaccurate, to an NRC licensee. The licensee subsequently submitted the inaccurate information to the NRC causing the licensee to be in violation of NRC regulations.

IALETTER 09/18/1998 On September 18, 1998, a letter was issued to the individual documenting the NRC's conclusion that he deliberately violated requirements when he compromised the integrity of an NRC reactor licensing examination. The letter also documented that the individual was convicted of a misdemeanor and that one of the items listed in his Plea Agreement was, ". . . defendant also agrees to never reapply for a position of employment with any facility under the jurisdiction, administration, or control of the NRC . . ."

IANOV 11/28/2007 On November 28, 2007, a Severity Level III violation was issued for a violation involving 10 CFR 55.53(j). Specifically, the individual, a licensed operator, performed licensed activities at Vermont Yankee, while he was under the influence of an illegal substance which could have adversely affected his ability to safely and competently perform those duties. Specifically, as part of Entergy's fitness for duty testing program, Mr. Picard was randomly selected for a drug and alcohol test on July 18, 2007. The subsequent results of that test were positive for marijuana.

IAORDER 08/02/2005 On August 2, 2005, an immediately effective Order Prohibiting Involvement in NRC-Licensed Activities (for five years) was issued to the individual based on his deliberate activities involving his possession of byproduct material without an NRC or an Agreement State license. The Order is effective for 5 years from the date of issuance of the Order. As a technician formerly employed by Professional Inspection and Testing Services, Inc., the individual maintained possession of a moisture density gauge without the licensee’s knowledge.
