Reporting ART Clinics for 2005 by State

Appendix C contains the listing of all reporting clinics. Due to the length of Appendix C it is broken down into 2 parts: A—M | N—Z.

If the clinic name has changed since 2005, the current name is listed in italics directly under the 2005 name.

Clinic names preceded by the § symbol have reorganized or closed since 2005. Reorganization is defined as a change in ownership or affiliation or a change in at least two of the three key staff positions (practice director, medical director, or laboratory director). Explanation of abbreviations for accrediting agencies used throughout this list:

CAP/ASRM = College of American Pathologists/American Society for Reproductive Medicine, Reproductive Laboratory Accreditation Program
JCAHO = Joint Commission on Accreditation of Healthcare Organizations
NYSTB = New York State Tissue Bank Program
