How do I file an IG complaint?
Please read and complete both forms; the DA Form 1559, Inspector General Action Request (IGAR); and DA Form 7433, Privacy Act Information Release Statement. You may continue your request on plain paper and please submit supporting documents.  Remember, sign the forms and return by email (preferred method), postal service mail, or fax the forms to RHC-C(P) IG.
