The Patient Administration Division is responsible for the completion of birth certificates of babies born at Bassett Army Community Hospital. Please remember when you have your baby and before time of discharge to bring your completed forms (Pink Form: Parent Worksheet for Preparation of Birth Certificates) to Patient Administration located in First Floor of BACH next to the LAB/Radiology. Fees:  The first copy of a certificate is $25 and $20 if any additional copies are ordered at the same time.
