Service members in the Warrior Care and Transition Program (WCTP) who separate or are released from active duty (e.g. physical disability separation, expiration term of service (ETS), punitive discharge under the Uniform Code of Military Justice (UCMJ), etc.) may need to update their DD Form 214. The DD Form 214 (Certificate of Release or Discharge from Active Duty) is a separation document summarizing a service member’s most recent period of continuous active duty service. The DD Form 214 contains information normally needed to verify military service for benefits and civilian employment opportunities. It is considered one of the most vital documents a service member will receive from the Army.

An incorrect DD Form 214 may cause delays in determining and processing your eligibility to receive certain benefits after military separation. Service members who identify errors or missing information on their DD Form 214, after the original has been delivered and/or copies distributed, should submit a written request to the appropriate agency for a DD Form 215 (Correction to DD Form 214, Certificate of Release or Discharge from Active Duty). The written request should include details about the items believed to be in error, including documentation to support the changes.

Service members who need a copy of their military service records can submit a request online, via mail, or by fax from the National Archives or via the eBenefits Portal .

Can corrections be made in the unshaded areas of the DD Form 214?

The Transition Center (TC) preparing the DD Form 214 may correct typographical errors in the unshaded areas before the Soldier’s departure and distribution of the DD Form 214.  Corrections must be neat and legible, and the authenticating official must initial each correction on all copies.

If I lose or cannot find my DD Form 214, where can I get a copy?

You can request it online and via mail or fax using an SF 180 by visiting the National Archives website. Another source to obtain your DD Form 214 is by going to the VA eBenefits Portal .
