Exceptional Family Member Program (EFMP)
The EFMP Office will be closed Wednesday October 19, 2016 for staff training. It will reopen on Thursday October 20, 2016.
The EFMP is a program for service members with qualifying family members with special needs. All branches of the military utilize EFMP and use the same enrollment process. Enrollment in the EFMP ensures maximum provision of services to the family throughout the sponsor’s career.
The EFMP office here at Walter Reed-Bethesda serves your family by:
Coordinates with other military and civilian agencies to provide medical, dental, mental health, developmental, educational, and community support to families with special needs.
Ensures your families special needs can be considered in the assignment process.
Refers your family to local resources to meet your special needs.
Helps your family with EFMP application process.
Coordinates Overseas Screenings to confirm the availability of medical and educational support at overseas locations.
About the Exceptional Family Member Program (EFMP)
The EFMP is designed to provide support to military family members with special needs. Special needs include any family member who is affected by a physical, mental health, developmental or educational condition, needs wheelchair accessibility or uses adaptive equipment and assistive technology devices and services.
Exceptional Family Members may be children or adults with any physical, developmental, intellectual, or emotional disorder that requires special treatment, education, training therapy, or counseling. Enrollment is due immediately upon diagnosis or identification of a special need. The DD Form 2792 should be completed by a clinical medical provider. The DD Form 2792-1 should be completed by the school or early intervention provider.
All packets (DD Forms 2792 and 2792-1) should be submitted during walk-in hours Monday between 6:30a.m-3:00 p.m.; Tuesday-Friday between 6:30 a.m. and 5:30 p.m. One of our office personnel will review the packet with you at that time to ensure it is complete. This is a medical legal document so it must be done in person. Packets can be submitted by EITHER the sponsor or the sponsor’s spouse.
**See the Forms Section on the right of this page for all Enrollment Forms and Instructions.**
Overseas Screenings
If you are selected to serve an OCONUS assignment, then an OVERSEAS SCREENING (OSS) must be completed on dependent family members by the EFMP office before family travel will be authorized and PCS orders are issued. Even if the family members do not require enrollment into the EFMP program an overseas screening must be completed for all OCONUS travel. OCONUS screenings are conducted on an appointment only basis (Walk-ins are not accepted). If you need to schedule an OCOUNS travel screen for your dependent family members please contact the MTF EFMP Coordinators. The EFMP coordinators will review your family’s medical and educational needs to ensure that the screening can be completed as quickly as possible.
The EFMP Coordinators are located at the medical treatment facility in the Pediatric Clinic, America Building (19), 4th floor, rooms 4430/4433. The EFMP Coordinators serve both Personnel and Family Support functions, but with an emphasis on the Personnel function.
