Congratulations on your assignment to Regional Health Command Europe!
If you haven’t already done so, complete the Sponsorship Program Counseling and Information Sheet, DA Form 5434 (DA Form 5434 or DA Form 5434.pdf) and send it to the RHCE HQ Program Manager.

Outbound Soldiers

One of the most important pre-move actions you can take is to request a sponsor. Please complete the Sponsorship Program Counseling and Information Sheet, DA Form 5434 (DA Form 5434 http://armypubs.army.mil/eforms/pureEdge/A5434.XFDL or DA Form 5434.pdf) and see your unit's sponsorship coordinator.
