Pre-Employment Forms

Excepted Service Candidates

 

 

 

The following Pre-Employment forms must be completed by every candidate for Excepted Service employment. These forms are fill-able and those requiring a digital signature can be signed using the “signature” feature in Adobe Acrobat ®. If you have not established a signature in Adobe Acrobat ® software, simply go to the “fill and sign” tools feature and select “place signature”. The software will guide you through the process. Each form should be saved on your computer as a separate .pdf file. When all required forms are completed attach them to an e-mail and return to your Central Processing Center (CPC) Representative. You may need to send several e-mails due to the size of the attachments. Please do not use a zip file, our system is having problems and this may cause your forms to be inaccessible. Please allow four (4) business days for a response to your submission.

1.     SF-86, Questionnaire for National Security Position (IF YOU HAVE A CURRENT SF 86 THAT IS LESS THAN 6 MONTHS OLD BUT NEEDS TO BE UPDATED, SEND IT AND USE THE SF 86C TO PROVIDE THE UPDATES.)

2.   SF-86C, Update to SF 86 – (Only Complete If You Have A Current SF 86 That Is Less Than 6 Months Old That You Will Send, But It Requires Updating. Otherwise Do Not Complete This Form.)

3.      SF 86 Quality Control Checklist – (This Is An Aid For You In Completing The SF 86 Form).

4.     DIA Form 418, Gender, Ethnicity, Race and Disability Self-Identification

5.      DD Form 2760, Firearms or Ammunition

6.      OF 306, Declaration for Federal Employment

7.      DIA Form 271, Conditions of Employment

8.      Civilian Mobility Agreement (Not Necessary To Complete Section 1)

9.      Notice of Random Drug Test

10.   Foreign Travel Form (If You Have Travelled Unofficially Within The Past 10 Years, You Must Submit This Form)

11.   Personal Financial Statement

12.   Selective Service Registration

13.   Foreign Contact Form – (Foreign Contacts That You Maintain A Personal Contact With [vs Professional] Must Be Reported. You Must Report All Social Media Foreign Contacts Including:  Facebook, Twitter, Linkedin, Snap Chat, Instagram, Etc. If You No Longer Have Contact With Those Listed On Your Social Media You Must Still Report Them But They Can Be Listed On A Blank Sheet Of Paper With Their Name, Email, Country And Any Other Information You May Have About Them. Make Sure You Put Your Name And SSN On The Top Of Each Page. These Contacts Must Also Be Reported On Your SF 86)

14.   Cohabitant Form(This Applies To Anyone You Are Living With That You Are Not Legally Married To Or If You Are Living At Home With Your Parents/Family Members Over The Age Of 18.)

15.   Dual Citizenship Letter

If you have attended college, please send a copy of your college transcripts. They may be official or unofficial.

Please send a copy of your Resume.

If you have served in the Military, please send a copy of your DD Form 214 (if applicable).

 

 


If You Are Married To A Foreign National, The Following Forms Must Be Completed:

 

 

 
      1.       Spouses Intent for US Citizenship

2.       Married to a Foreign National Form

3.       Candidate Foreign National Letter (see sample)

4.      A listing of your spouse's first degree relatives (immediate family members) to include their full name, relationship to them, date of birth, place of birth, residential address, occupation, and address of employer. (We have a foreign contact report for you mother and father in-law). This is a different requirement that we must have from the applicant.

5.       A copy of your spouse's passport or birth certificate with a certified English translation of the document.

6.       Personal Letter addressing the following:

                a. Signed statement as to your spouse's intentions of becoming a US citizen;

                b. Detailed circumstances under which you met your spouse;

   c. The inclusive period of association, frequency of contact and the conditions under which contact has been accomplished.

   d. Exact nature of association.

      7.    Signed and dated SF86 from the spouse

Wounded Warrior Candidate
