Policies
We are a fragrance-free clinic. Please refrain from wearing perfume or cologne to your appointment.
Please call the clinic if you will not be able to keep an appointment so another patient can be scheduled. If you no-show for your appointment, please call and reschedule as soon as possible. Three no-show appointments could result in discharge from the clinic.
Please arrive at New Patient Registration (located on the 1st floor of the Patient Care Center) 45 minutes prior to your appointment time.
Please bring your insurance card and a picture identification card.
If an authorization is required for an appointment, it is the patient’s responsibility to obtain this from the Primary Care Physician (PCP) prior to the appointment date. If the patient has not obtained a referral at the time of the appointment, the patient will be responsible for total services rendered.
