Flu Vaccines
The flu vaccine is still available for beneficiaries.

Pertussis Booster
All Army need a Pertussis Booster by mid-February. If you are unsure of your status, please contact the clinic or your unit's MEDPROS representative.

Why Do I Need a Flu Vaccine Every Year?
A flu vaccine is needed every year because flu viruses are constantly changing. It's not unusual for new flu viruses to appear each year. The flu vaccine is formulated each year to keep up with the flu viruses as they change.
Also, multiple studies conducted over different seasons and across vaccine types and influenza virus subtypes have shown that the body's immunity to influenza viruses (acquired either through natural infection or vaccination) declines over time.
Getting vaccinated each year provides the best protection against influenza throughout flu season.

Physical Exams


Child and Youth Services (CYS) Physical:

Please call the central appointment line for a provider appointment.

Bring these forms with you:
CYS Physical Form
CYS Health Screening Form
Asthma Action Plan (if your child has asthma)
CYS Allergy Action Plan (if applicable)
CYS Special Dietary Needs (if applicable)

School (College) Physicals:

Ensure that you have teh correct form(s) from your school and call central appointments for a provider appointment.

Employment Physical:

The form(s) for your employment physical are specific to your employer. Please obtain these from your respective employer, fill out as much of the information about your medical history as possible and then make an appointment with a provider. Please be aware that if extra tests are needed.

Army PHA (Yearly Requirement):

Ensure that the online portion is completed through AKO, then call the central appointments line and get a PHA appointment. The medic will go over your forms with you and forward them to the provider. If you need provider intervention, that will be set up for you at this appointment.

Air Force PHA (Yearly Requirement):

Ensure that you have the correct questionnaire and that it is filled out. The clinic can provide this as well. Then call the central appointments line and get a PHA appointment. The medic will go over your forms with you and forward them to the provider. If you need provider intervention, that will be set up for you at this appointment.

Military Physical:

Separation, initial entry, green-to-gold, special forces, airborne, air assault, ranger, sniper, pathfinder, firefighter, sergeant major academy physicals can be done by filling out the DD Form 2807-1, MAR 2007 (Report of Medical History) and the demographic data on DD Form 2808, OCT 2005, (Report of Medical Examination) and calling the central appointment line to make an appointment for Part I of your physical with a medic. If you have a limited duty profile, bring this to your visit. During the visit, the medic will determine what, if any, special tests are required and order them. You can then make an appointment for Part II of your physical with a provider who will finalize and sign both forms.

Flight physicals
These physicals are not performed at US Army Health Clinic, Livorno. Please call 632-5101 at Aviano Health Center to schedule a flight physical at their center. This is the closest location to Camp Darby which provides this service.
