- Complete contact information of requester (name, address, state, zip, country, email, phone, and fax);
- USIP Press title information (name of book, author, ISBN, chapter title /author where applicable, and page numbers);
- and, information about the proposed publication and specific rights requested (title, author/editors,
publisher, formats requested, relevant prices and ISBNs, page count, pub date,
print run(s), distribution territory, languages requested, etc.).
