Physical Therapy provides the highest quality physical therapy health care to all eligible TRICARE beneficiaries, provide injury prevention and rehabilitation consultation to Fort Lee active duty units, provide clinical education to affiliated physical therapy students and ensure continued professional development of all physical therapy staff members.

Physical therapy focuses on improvement in function over time.  Initial evaluation and follow-up evaluation includes use of the following questionnaires.  If you are being referred to physical therapy for:

 

Upper back or neck pain - please fill out Neck Disability Index Survey

 

Low back pain  - please fill out Oswestry Low Back Pain Disability

 

Shoulder pain - please fill out Shoulder Pain and Disability Index 

 

Hip, knee, leg or ankle pain -please fill out Lower Extremity Function Survey

 

Bring the completed questionnaire to your physical therapy visit to save time once you arrive.   If you are not able to print it out, we will provide you with a survey to complete before your visit.

Self-care/common condition information for supplementing home exercise program instructions for reveiw as needed.
