• Special Power of Attorney for Children of DoD Parents who are to remain behind (children must have specific powers of attorney for other adult evacuees to escort them to the designated safe haven and turn them over to the designated guardian
• Copies of PCS Orders and Area Clearance
• Family Care Plan
• School Records
• Important Okinawa and U.S. Phone Numbers (family members, doctors, emergency numbers, etc)
• Employment Documents (SF 50 Notification of Personnel Action for government employees, most recent performance appraisal, etc)
• Housing Documents (housing contract, inventory, etc)
• Automobile Documents (bill of sale, registration, etc)
• Credit Cards, Checkbook and $100 Minimum Cash (Yen and USD)
• Pet Records if applicable (DD Form 2209 Veterinary Health Certificate, Kadena pet registration, immunization records, rabies vaccination certificate, etc)

Keys:

• Residence Keys with DD Form 754 Repair Tag
• Automobile Keys with DD Form 1864 Vehicle Key Tag

Three-Five (3-5) Day Supply of:

• Water (one gallon per person per day)
• Non-perishable, High-Energy Food (non-electric can opener if carrying cans)
• Baby Food/Formula
