Sick call sign-in is Mon-Wed & Fri from 0630-0830, Thurs 1000-1030 & 1300-1330.
Closed on Federal Holidays.
Bldg 649 New Guinea Road, Fort Story, VA 23451 Only Active Duty service members assigned to Fort Story Health Clinic. All appointments will be made directly with the Fort Story Health Clinic. Appointments are not made through the Hampton Roads Appointment Center for this clinic. All physicals including Periodic Health Assessments (PHAs) must done in uniform.
ETS physicals need to be scheduled 180 days before your ETS date.
Physicals, to include well woman exams, are by appointment only.
Hearing tests are by appointment only. You must bring in your medical records if you have them and an ID card. Units that require mass hearing tests need to call the Fort Story Health Clinic to have a date and time established with the Hearing Conservationist. All in and out processing will be done after 1000 hours daily.
Be sure to bring your in-processing paperwork, military ID, a copy of your orders and your medical records to be signed into the clinic. Soldiers do not hold on to their medical records.
Hours for labs, are Mon-Fri between 0800-1030. No labs will be accepted after 1100 to allow for sufficient time for lab processing and pick-up. All walk-in immunizations will be done on Mon-Fri between 0800-1100 and 1300-1445. PPDs will not be administered on Thurs or Fri. Units requiring mass immunizations need to call for further assistance.
