- Contact your school to obtain their required forms

Military dependent ID Card, if 10 years of age or older, PRIME to KACC

Medical Records, if not maintained at Kimbrough

Shot Records (Add) Please have shot records pre-screened in Immunization

Clinic prior to appointment during normal duty hours.

Third Party Insurance Card (Everyone must complete the DD Form 2569)
Proof of Guardianship, if you are not the parent of the child being seen

Due to safety concerns, children without appointments are not permitted
in clinics.
