Coming to Zama American HS soon? Try our DoDEA Online Pre-Regsitration for incoming families to get your child(ren) pre-registered. You must still complete the registration process when you arrive.

New Student

Sponsor fills out the following documents:

• DoDEA Student Registration (Form 600)
• The sponsor must sign the Form 600. If the sponsor is unavailable the spouse may sign.
• Student Health History (DoDEA Form 2942.0)
• Please include any special health concerns necessary for your child's safety and well being.
• DoDEA Form 700 Consent

Sponsor presents the following:

• Student's proof of birth (copy of the birth certificate, passport, or dependent ID card)
• Student's immunization record
• If military, copy of sponsor's orders or Command Sponsored Letter. Area clearance message or dependent entry approval message listing school age dependents by name. (Navy or Marines)
• If civillian, Letter of Employment, or DD 1614 orders
• Area clearance message or dependent entry approval message listing school age dependents by name. (Navy or Marines)

Returning Student

Sponsor or spouse fills out the following documents:

• Sponsor/Pupil Registration (DoDEA Form 600)
• Update for Student Health History (DoDEA Form 2942.0)
• Present an updated copy of sponsor's orders,Letter of Employmentif needed. This is only necessary if the sponsor has received an extension of the original orders, or if the sponsor has been assigned to a different command within Zama/Atsugi commuting area.
