New Student

Sponsor must fill out the following documents online. Then print documents and bring to registrar:

• Sponsor Registration DoDEA Form 600: Signed by sponsor, spouse or guardian
• Questionnaire for Race/Ethnicity and Home Language DoDEA Form 600B and DoDEA Form F4
• Consents and Authorizations DoDEA Form 700
• Education Pre-Screening Questionnaire DoDEA Form 620
• Computer and Internet Access Agreement for students DoDEA Form 6600.1
• Student Health History DoDEA Form 2942.0-M-F1

Sponsor must present:

• Permanent Change of Station (PCS) Orders, Verification of Civilian Employment DoDEA Form 602 or Contract Extract for Defense Contractors
• Dependent’s connection to sponsor (if not covered in PCS Orders or Employment Verification); official memo or correspondence that lists dependent by name (Command Letter, Military Personnel Memo, Dependent Entry Approval)
• Student's immunization certificate

Returning Student

Sponsor or spouse must fill out the following documents:

• Sponsor Registration DoDEA Form 600-preprinted by the school)
• Returning Student Health History Update DoDEA Form 2942.0-M-F2
• Present an updated copy of sponsor's orders if needed. This is only necessary if the sponsor has received an extension of the original orders, or if the sponsor has been assigned to a different command within the commuting area.
