The ONRR can only make changes to official Address and/or Addressee of Record information upon receipt of a signed, original Form ONRR-4444. Open and read the Instructions document as you complete the form.

Company Name Change Information
Company name changes must be initiated by the company through the BLM (for onshore Federal leases), the ONRR in New Orleans (for offshore leases) or the BIA (for Indian leases). The company and the ONRR will be notified in writing that the name change is recognized and the effective date.
