Overview This section covers the key elements needed for providing continuous proactive care for patients with diabetes: patient-centered care, up-to-date clinical management, team care, and community partnerships. They are all components of the models of care identified in the "How" section. To determine where your practice or organization currently rates concerning these elements, you may want to read about assessing needs and setting priorities in the "Needs" section. Although each practice and organization will undoubtedly have different goals, some may decide that it is top priority to implement one or more of these elements. Goals may need to be addressed in phases. A long-term approach with continuous review and ongoing modification will ultimately produce meaningful results. Keep in mind that successful implementation may depend on your having an established framework of evidence-based decision-making and an information system that allows you to identify groups of patients. Those topics are covered in the "Framework" section and should be included in any system-change initiative. Suggestion This section on "What" is one of the largest in BetterDiabetesCare.
