Passport and Travel Resources

US Citizens must have a current passport to travel outside of the United States, including Mexico and Canada. Passport cards are now available for land and sea border crossing, but all international air travel requires a passport book.

 

You may apply for a passport at your nearest passport acceptance agency, locations and forms can be found at the State Department's website.

 

If you are traveling within two weeks you may request an emergency appointment at the Chicago Passport Office, located at 230 South Dearborn Street, by calling (877) 487-2778. Proof of travel is needed for emergency appointments.

 

Applications for new and expired passports require the necessary form, proof of citizenship, two passport photos, applicable fees, and a current state issued photo ID. There is a charge of $60.00 to expedite a passport. Forms and fees are also available at the State Department's website.

 

Passport applications for children under the age of 16 require the consent of both parents. If only one parent is available at the time of application, written and notarized consent as well as a copy of current state-issued ID of the parent not attending is necessary. The child must be present to submit the application.

 

Lost or stolen passports should be reported to the Department of State at (202) 955-0430 as soon as possible. If a passport is lost while traveling abroad please contact the nearest US Embassy for a temporary passport.

 

You may complete a Privacy Act Release Form if you would like us to inquire into a pending passport application, include your date and place of travel, and fax it to the Senator's nearest State office.

 

The Department of State highly recommends US Citizens register with the Department when traveling overseas.
