EA-10-080 Calvert Cliffs 1 & 2	NOV (White)	08/03/2010	On August 3, 2010, the NRC issued a White Significance Determination finding with an associated violation to Calvert Cliffs Nuclear Power Plant (Calvert Cliffs).  This White finding involved the licensee’s failure to develop and implement scheduled preventive maintenance for Agastat E7000 series time delay relays, as required by Technical Specification 5.4.1.  Specifically, subsequent to the approval of Engineering Change Package No. ES200100067, issued in March 2001, the licensee did not replace the relays within the vendor recommended 10-year lifetime, nor establish a performance monitoring program.  Consequently, on February 18, 2010, an Agastat E7000 series time delay relay that had a lifetime in excess of 10 years, used in the 2B emergency diesel generator (EDG) protective logic, timed out early and failed to support a demand fast start and run of the 2B EDG.  This resulted in the EDG becoming inoperable with the resultant loss of alternating current to the 24 safeguards bus during the dual unit trip that occurred on February 18, 2010.
EA-08-351 Calvert Cliffs 1 & 2	NOV (White)	04/03/2009	On April 3, 2009, a Notice of Violation (NOV) was issued to Constellation Energy for a violation associated with a White Significance Determination Process finding at Calvert Cliffs.  Specifically, the licensee failed to maintain in effect Emergency Plans that met the requirements of 10 CFR 50.47(b)(4) and 10 CFR 50, Appendix E.  An emergency action level table used by operators to assess the functionality of the containment barrier during an accident contained an inaccurate threshold for identifying a potential loss of the containment barrier.  This error could have adversely impacted the licensee’s ability to accurately classify an emergency condition.
EA-06-198 Calvert Cliffs 1	NOV (White)	10/27/2006	On October 27, 2006, a Notice of Violation was issued for a violation associated with a WHITE significance determination process finding involving inadequate design control during the establishment of the over-current trip setting for an electrical circuit breaker that supplies power to the support systems for the 1A emergency diesel generator (EDG). The violation was cited against 10 CFR Part 50, Appendix B, Criterion III, "Design Control," because the licensee failed to ensure that design control measures provided for verifying or checking the adequacy of the design when Calvert Cliffs modified the on-site electrical distribution design by installing a new 1A EDG in 1996.

From November 1, 2007, through November 21, 2007, while the plant was in Mode 1, one of the two DGs capable of supplying the onsite Class 1E power distribution subsystem(s) was inoperable, and action was not taken to either restore the DG to an operable status within 72 hours or be in Mode 3 within 6 hours and Mode 5 within 36 hours. Specifically, Emergency Diesel Generator (EDG) 1-02 was made inoperable as a result of painting activities due to paint having been deposited and remaining on at least one fuel rack in a location that prevented motion required to support the operation of the EDG. This condition caused EDG 1-02 to fail to start during a surveillance test on November 21, 2007.

Specifically, Violation 05000298/2008008-1, dated June 13, 2008, identified a condition adverse to quality in that two procedures would not work as written. While correcting that violation, the licensee failed to perform sufficient evaluation of the circuits to identify and correct a problem with three motor-operated valves needed to establish core cooling. Failure to correct the condition adverse to quality resulted in inadequate procedures in that they contained steps that were inappropriate to the circumstances because they would not work as written to reposition the three motor-operated valves.

EA-07-204 Cooper	NOV (White)	06/13/2008	On June 13, 2008, a Notice of Violation (NOV) was issued for a violation associated with a White Significance Determination Finding involving a violation of 10 CFR Part 50, Appendix B, Criterion V, “Instructions, Procedures, and Drawings.” Specifically, between 1997 and June, 2007, the licensee failed to ensure that two emergency operating procedures which were used to bring the plant to a safe shutdown condition in the event of certain postulated fire scenarios would work as written. Additionally, the licensee failed to properly verify and validate procedure steps to ensure that they would work to accomplish the necessary actions.

The licensee failed to establish measures to promptly identify and correct a significant condition adverse to quality (SCAQ) and failed to assure that the cause of a SCAQ was determined and corrective action taken to preclude repetition. Specifically, the licensee’s inadequate procedural guidance for evaluating the suitability of parts used in safety related applications presented an opportunity in which the licensee failed to promptly identify a defective voltage regulator circuit board used in Emergency Diesel Generator (EDG) 2 prior to its installation. Following installation of the defective EDG 2 voltage regulator circuit board, the licensee failed to determine the cause of two high voltage conditions, and failed to take corrective action to preclude repetition. As a result, an additional high voltage condition occurred resulting in a failure of EDG 2.
