Patient Centered Medical Home (PCMH) PCMH is a new approach to healthcare delivery based on the concept that the patient is always at the center of every decision made regarding their medical care. PCMH provides a continuous relationship with a personal physician and team coordinating care for both wellness and illness management. Fox Army Health Center's PCMH is complied of three teams, Red, Blue and White. Thank you for making Fox your Medical Home. Fox Army Health Center 2012 Flu Shot Schedule Active Duty (A/D), National Guard, Reservists (all Services): 6 Nov (Tues) 1330-1530, Preventive Medicine, Fox Army Health Center MILITARY RETIREES AND THEIR ADULT FAMILY MEMBERS ONLY – RETIREE HEALTH FAIR 28 September 2012, 0730-1130 AT THE OVERLOOK AT REDSTONE ARSENAL (Note: this will be a drive thru flu shot event and shots will be administered to (ADULTS 17 and OLDER ONLY) ALL Beneficiaries (A/D and their family members, military retirees and their family members of all ages): Tricare Prime with providers at FAHC will be offered flu shots during scheduled appointments beginning 1 Oct 2012. All other beneficiaries may receive shots as scheduled below: 7 Nov (Wed) 1600 - 1800 at Fox Army Health Center (all ages) 16 Nov (Fri) 0900 - 1300 at the Redstone Arsenal Commissary (adults only) Flu shot screening forms and Vaccine Information Statements for adult and pediatric vaccines will be available for printing beginning 19 Sep from the Fox Army Health Center website at www.redstone.amedd.army.mil ![]() For more information, please call 955-8888, x1441 or 1442. Renovation Project at Fox The first phase of Fox Army Health Center’s heating, ventilation and air conditioning (HVAC) system renovation started Friday, September 7, 2012. Trailers in the east end parking lot currently house the Phase 1A/1B occupants: Wellness Clinic, Preventive Medicine, Occupational Health, and Medical Records. Directional signs are posted to help beneficiaries navigate the facility through the renovations, which are estimated to be completed in October 2014. As a reminder, the following are the estimated phases of the project, at which specified clinics will move to the transitional trailers in the parking lot. Each phase is estimated to take 45 to 60 days, which is subject to change. Phase 2: Patient Centered Medical Home (Red, White and Blue Teams) Phase 3: Command Suite Phase 4: Radiology and Quality Management Phase 5: Optometry and Physical Therapy Phase 6: Behavior Health Department Phase 7: Managed Care Phase 8: Logistics, Industrial Hygiene, Facilities, and Housekeeping Phase 9: Pharmacy, Laboratory, and Dental We apologize in advance for the inconveniences of this project; however, patient care will not be impacted. Fox Army Health Center Schedule Update As a Reminder, the Health Center is closed on the first Thursday afternoon of every month, Saturday, Sunday, and all federal holidays. Advice Nurses are available during non-duty hours at (256) 955-8888 or 1-800-223-9531 to provide care advice. Fox is an ambulatory care clinic and does not have an emergency room. In an emergency, dial 911.Optometry Clinic Update The Fox Army Health Center's Optometry Clinic is currently operating with one Optometrist on every Monday and Thursday and two Optometrists on every Tuesday. Active duty Soldiers are a priority on these days and all others beneficiaries are space available until further notice.
