You may submit information electronically via Centurion (our secure online reporting system), by mail or fax.

Use the links on the left to view the current forms used to collect data for the survey. All forms are in Adobe PDF file format.


Please Remember - Include company name and ID on the survey cover page. Call 1-800-227-1735 if assistance is needed to determine company ID and the industry category codes in which your company operates.

Questions - Refer to the FAQs with any questions regarding these report forms. Direct any additional questions to 1-800-528-3049.

Filing the Report Form - Return your completed report form to the U.S. Census Bureau, 1201 East 10th Street, Jeffersonville, IN 47132-0001 or fax it to 1-800-438-8040. Make a copy of the completed report form for your company records.
