This training will discuss the guidelines to address progression and co-morbidities which affect chronic kidney disease (CKD) outcomes. Current adherence to the guidelines, models for improving outcomes and opportunities for intervention will be described.

CKD Nutrition - This training includes a review of the functions of the kidneys, identification and monitoring of CKD and describes new tools from the National Kidney Disease Education Program (NKDEP) to enhance patient education.
CKD Screening - This training will cover several areas including definitions, screening, diagnosing and monitoring of chronic kidney disease and preview tools for patient education.
Patient Education Videos
– Short videos demonstrating patient-provider interactions around common CKD questions. Topics include CKD and risk, treatment, testing and diagnosis, and dialysis and transplantation.
