Medical Requirements for DoD Civilian Employees

Medical Prerequisites Checklist

Employees must complete a thorough medical examination no earlier than (NET) 90 days prior and no later than (NLT) 30 days prior to deployment date (D-date) at home station to determine medical, psychological and physical fitness for deployment in accordance with the CENTCOM medical standards CENTCOM medical standards. DoD Civilians deploying to CENTCOM's area of responsibility must meet the medical requirements outlined in MOD 11 to CENTCOM, USCINCCENT Individual Protection and Individual/Unit Deployment Policy and TAB A to Mod 11, Amplification of the Minimal Standards of Fitness for Deployment to the CENTCOM AOR (See MOD 11 U.S. CENTCOM Individual Protection and Individual-Unit Deployment Policy).

  • All identified medical issues and abnormal test results must be specifically addressed by the servicing physician on the DD Form 2808 and/or require further testing.
  • Fitness includes, but is not limited to, the ability to accomplish the tasks and duties unique to a particular operation, and the ability to tolerate the environmental and operational conditions of the deployed location, including wear of protective equipment and use of required prophylactic medications.
  • Employees must be found fit for duty and fit for the specific deployment without limitations or need for accommodation by a medical and dental evaluation.
  • Final medical deployment eligibility determination lies with the clinician providers assigned to the deployment center.
  • The medical waiver approval authority for CENTCOM medical standards is the CENTCOM Surgeon Office.
  • Medical examination must be completed at home station and forwarded to the deployment center NLT 14 days prior to scheduled attendance. Incomplete medical information or having a certain medical condition is the primary reason an individual will not deploy or is delayed. Employees must bring copies of all medical documentation with them to the deployment center as part of their deployment packets.

 The medical examination consists of the following:

  •  DD Form 2808 DD Form 2808, Report of Medical Exam, completed and signed by your doctor.
  •  DD Form 2807-1 DD Form 2807-1, Report of Medical History, completed and signed by your doctor.
  •  DD Form 2795, Pre-Deployment Health Assessment (needs to be filled out electronically).
  •  DD Form 771 DD Form 771, EYEWEAR PERSCRIPTION, must be completed and returned, so that your protective mask and ballistic glass inserts can be ordered. This form MUST ALSO be included in your physical packet, even if glasses/contacts are not required as determined by medical authority. In that case, simply write "glasses not required" on the form. The DD 771 is the only form accepted by DoD when ordering prescription eyewear. NO OTHER FORM OF PRESCRIPTION CAN BE ACCEPTED. (You can include your civilian prescription(s) with the DD 771 and must include measurements.) To avoid delays in the ordering process, please ensure all writing is clear and legible. In order to meet deployment requirements, ALL civilian personnel who require corrective eyewear are required to have two sets of prescription glasses prior to deployment and will be issued at the deployment center.
  •  DD Form 2215 DD Form 2215 Reference Audiogram or DD Form 2216 DD Form 2216 Periodic Audiogram in their medical record completed within the past twelve months.
  • Spirometry (Pulmonary Function Testing) results and OSHA Respiratory Medical Evaluation Questionnaire form OSHA Respiratory Medical Evaluation Questionaire.

Copies of the laboratory reports of the following tests completed: 

    • Urinalysis (Routine), includes testing of color, Sp Gravity, PH
    • Comprehensive Metabolic Panel (CMP) (includes a hemoglobin A1C if you have diabetes, are glucose intolerant (high blood sugars), or are taking medication to control blood sugar).
    • CBC (Complete Blood Count)
    • LIPID Profile (Age 40 or above, must fast 8-12 hours, may drink water)
    • EKG (Age 40 or above) If abnormal, must complete additional cardiac studies.
    • Blood Type/RH
    • DNA on file by the MTF doctor
    • A negative HIV test result within the last 120 days of deployment
    • Glucose-6-Phosphate Dehydrogenase Test (G6PD). The test measures the amount of G6PD, an enzyme that protects red blood cells from the effects of oxidation
    • Copy of Audiogram
    • Spirometry (Pulmonary Function Testing) results and OSHA Respiratory Medical Evaluation Questionnaire form, needed only if you have a history of Respiratory problems such as Asthma, Chronic Obstructive Pulmonary Disease (COPD), or if the civilian employee is a heavy smoker
    • Blood in Urine (Hematuria) test
    • Automated Neuropsychological Assessment Metric (ANAM) within 12 months prior to deployment
    • Glaucoma test
    • For those who are in H-3 Hearing Test Category a SPRINT Test is recommended within 20 days of deployment

 Females

  • Pregnancy Test (Urine), within 30 days of deployment. Women who have had hysterectomies, had a bilateral tubal ligation, or who are post menopausal are exempt.
  • There could be certain circumstances, such as previous abnormal tests or family history issues where these time limits could vary.

 Immunizations: Employee should attempt to obtain all immunization prior to scheduled movement to the deployment center. However, those unable to receive or obtain all required immunizations prior to arriving at the deployment center will receive all undocumented immunization at the deployment.

  • ANTHRAX Anthrax vaccination is required for all CEW volunteers deploying to the CENTCOM AOR, but is not given to personnel 65 and older. Highly advisable to get first two immunizations or subsequent dose/booster prior to deployment.
  • HEPATITIS A This is a 2 part series: day 0 and 6 months later.
  • HEPATITIS B This is a 3 part series: day 0, 30 day and 5 months after 30 day
  • TWINRIX (combination Hep A & Hep B)- 3 part series: day 0, 30 day and 5 month after 30 days
  • INFLUENZA (seasonal) October- June (This is not optional)
  • MMR (Measles, Mumps, Rubella) Given 1 time over the age of 18 years
  • POLIO Given 1 time over the age of 18 years
  • TETANUS/DIPHTHERIA Within the last 10 years
  • TUBERCULIN SKIN TEST Within the past year (with documented result). Civilians who have had a positive reaction from the Tuberculin skin test, and provide documented, will not receive another
  • TYPHOID Within the last 2 years
  • SMALLPOX Civilians will be evaluated by pre-deployment medical staff for the smallpox vaccine. Civilians who are able to receive the vaccine will be given written instructions as to how to take care of the vaccine. For more information on Smallpox: www.smallpox.mil , www.vaccines.mil. May be administered up to 120 days prior to deployment. Not given to personnel 65 and older
  • VARICELLA Required documentation of one of the following: history of disease, sufficient varicella titer, or administration of vaccine.
-Allergy Identification Tags (Red) for all allergies (if applicable).
-Bring a 180 day supply of all prescription medications; recommend bringing sufficient amounts of any over the counter medications need also.

Dental Prerequisites Checklist

Employees must bring copies of all dental documentation with them to the deployment center as part of their deployment packets.

Orthodontic appliances do not preclude deployment eligibility provided they are evaluated for stability and inactivated through the use of passive holding arches and secured with stainless steel ties or other means prior to deployment.
