    Changes to any official Address and Addressee of Record information can only be made by the MMS upon receipt of a signed, original Form MMS-4444. The instructions are specifically for Payors and Operators.

    Company Name Change Information
    Company name changes must be initiated by the company through the BLM (for onshore Federal leases), the MMS in New Orleans (for offshore leases) or the BIA (for Indian leases). The company and the MMS will be notified in writing that the name change is recognized and the effective date.
