A: The defendant may request
defensive driving either in person or by
mail. We CANNOT accept telephone
requests. If applying by mail, please
enclose a self addressed and stamped
envelop, a notarized Letter of Request,
a copy of the defendant's current
insurance and driver's license, and a
check or money order in the amount of
$107.00 ($132.00 if violation occurred
in a school zone).
