RADIATION CONTROL PROGRAM

Laser and Laser Services Registration

For a new Certificate of Laser Registration, please submit the following along with the appropriate fee:

• RC Form 301-2 - Application for Registration of Lasers and Laser Services. 
  Only Class IIIb or Class IV Lasers need to be registered.
  
  
• Laser Safety Officer(LSO) qualifications.
  Submit documentation of laser experience, education and/or training.
  If a Physician signs as LSO no additional qualifications need to be submitted.
  
  
• Laser Safety Training Policy.(For Human Use Applications Only)
  Please submit a policy of the laser safety training provided or required for operators of lasers as applicable for the type and class of the laser device. Training shall include but not be limited to safety measures taken to ensure safe operation of equipment to minimize danger to the operator and to the patient. These measures shall include:
  · Use of proper protective eyewear.
  . Proper signage, labeling, posting, and barriers.
  · Users are familiar with different skin types in order to best determine appropriate settings required for safe use.
  · Users are familiar with different medications in order to best determine appropriate settings required for safe use.
  · Operating and Safety Procedures and operator’s manual readily available.
